Inspirational Journeys: From Surgeons to Audiologists We are inspired by the dedication of our cochlear implant (CI) recipients, who thrive in their professional roles. 🌎 Jason, an American surgeon with genetic hearing loss, shares, "Hearing loss disconnects you from the world and the people around you. My world got smaller as my hearing worsened." 🌍 Mercedes, a Spanish radiologist with sudden hearing loss, hyperacusis, and tinnitus, reflects, "These conditions [hyperacusis and tinnitus] were limiting factors in my return to family, leisure, and work life." 🌏 Helen, an Australian audiologist with Ménière's Disease, advised, "Don't underestimate the difficulties of single-sided deafness. 2024 research finds improved sound localization with bilateral middle ear implants. A recent article in Trends in Hearing reveals that bilateral MEI users experience significant improvement in sound localization, with some achieving near-normal performance. Right place, right time. Both are essential for cochlear implant sound quality. What do we mean by this? In natural rate coding, the nerve signal timing is locked to the soundwave timing. If an incoming sound has a frequency of 125 Hz, the hair cell at the 125 Hz location in the cochlea triggers 125 nerve signals per second. The brain precisely understands this as a 125 Hz sound. With a CI, if the same sound is stimulated at the right place in the cochlea but with a standard fixed stimulation rate of 800 electrical pulses per second, the brain will understand this as an upward shift in pitch closer to an 800 Hz sound signal. This causes “pitch confusion.” MED-EL’s long electrodes reach the widest range of natural frequencies beyond one and a half turns in the cochlea. Here, rate coding is vital for stimulating lower frequencies to provide a hearing experience closest to natural hearing. What does the color coding in impedance field telemetry (IFT) heat maps in MAESTRO 10 indicate? These heat maps reveal essential data about how much current from one contact or channel reaches another. Here’s a quick rundown: • Lighter colors indicate higher current flow. • Darker colors represent lower current flow.
