It was a privilege to present at the Australian Tactical Medical Association Conference on Occult Ketamine Exposure Among First Responders. Ketamine use in the battlefield has sharply increased since its introduction into TCCC guidelines in 2011. First responders and military medics may face hidden exposure risks to ketamine we're only beginning to understand. This message tied into a broader theme during the conference of evolving best practices in the care provided by first responders. Patrick Weinrauch's talk on Tourniquet Ischaemia in Prolonged Field Care provided a compelling parallel. His presentations on the tragic amputations in the Ukraine war due to prolonged tourniquet application was deeply disturbing. He highlighted that the tourniquet application culture needs reassessment with the key message being that continuous re-evaluation is crucial in the world of irregular warfare and that tourniquet application de-escalation may be safe in certain situations. More reading on this can be found here - https://lnkd.in/gzVbEAwR As we gain more tools to improve patient care, we must continuously refine our understanding of their safe and thoughtful use. Whether it's ketamine or tourniquets, we can't afford a culture of paranoid practice. Instead, we need to have ongoing learning from past experiences and broaden our understanding of equipment uses, limitations, and potential adverse effects with continual refinement of training and education.
