The Working Better for Medicare Review was released this month, and our team have reviewed the 160-page document and provided an overview of what we feel are the key recommendations that medical centre operators need to be aware of from the report. It is important to note that these are recommendations only at this stage and there is no date for a decision on if and when these proposals will be adopted. Most notable is the proposed overhaul of DPA regions and a reduction in 19AB exemptions. These include limiting locum exemptions to one per GP per moratorium. These proposals, if enacted, would significantly reduce the benefit of the new expedited pathway. Although, and this is only the opinion of our team, we do not feel that the expedited pathway will significantly increase the number of GPs relocating to Australia. More on that in another post. Executive Summary (Pages 2-6) Key recommendations # Governance #1: Establish a Health Workforce Independent Review Panel. 19AA #2: Retain Section 19AA to maintain training and practice standards. #5: Provide financial and professional support for all GP registrars. #6: Align supervision payment with the National Consistent Payment Framework. 19AB #7: Retain Section 19AB to address workforce maldistribution. #8: Clarify and tighten Section 19AB exemptions for spousal, academic, locum, afterhours & replacement Limit locum exemptions to one per GP per moratorium #9: Strengthen monitoring of Section 19AB exemption compliance. Recommendation 10: Maintain the 10-year moratorium but incentivize practice in high-need areas. #11: Improve IMG training, supervision, and support under Section 19AB. DPA #12: Retain a DPA mechanism to distribute GPs to high-need areas (rather than areas of GP workforce shortage) #13: Base DPA status on GP catchments, not MMM. #14: Revise DPA methodology with the National Assessment Tool (NAT). Recommendation 15: Review DPA status annually with processes for non-DPA areas. #16: Align Bonded Medical Program ROSO with regional and remote areas. Complementary Strategies #20: Set a national target for medical workforce self-sufficiency reducing dependency on IMGs #21: Align medical student placements with regional workforce needs. #22: Support postgraduate training for rural care providers including RANs & Midwives. #23: Coordinate migration strategies with health workforce needs. #24: Use DPA levers to prioritize underserved areas. #25: Align strategies with the Closing the Gap reforms. #26: Enable IMGs to fill unfilled training positions in Aboriginal Health services. ⚡ The new GP Expedited Specialist Pathway (ESP) has officially opened as of the 21st of October. ⚡ As with any new GP training program, there has been many questions. Initially this new pathway got me very excited but now I’m not too sure. Over the past few years, I have successfully helped many SIMGs relocate to Australia on the PEP Specialist Stream and the ACRRM Specialist Pathway. The process is long, expensive, tedious and the application process is complex. But in the end, the objective was to acquire Australian GP Fellowship. The ESP will be taking a different approach to save time. Let’s take a closer look…. Currently, you must hold one of the following specialist qualifications to be eligible: 💠 MICGP (since 2009) with CSCST 💠 MRCGP (since 2007) with CCT 💠 FRNZCGP (since 2012) Fellowship: 🏆 The ESP will bypass the RACGP or ACRRM to allow SMIGs quick access to AHPRA registration. The ESP will not lead to GP Fellowship. SIMGs can choose if they want to apply to RACGP/ACRRM for Fellowship and complete the college requirements whilst on the ESP. It is also worth noting that General Registration will not be automatically granted. You will need to submit another application to AHPRA after 12 months to apply for General Registration. Fees: 💸 AHPRA application fees are substantially more than applying for Provisional Registration. ESP will cost $2575; compared to $1226. SIMGs will be responsible for Work Base Assessment fees and multi-source feedback assessment fees. There may be other assessment costs depending on the type of assessment. SIMGS choosing to gain Fellowship will also need to pay additional college fees. These figures are currently unknown. CPD: 💪 AHPRA CPD registration standards applies to all applicants. SIMGs will still need to pay an annual RACGP/ACRRM membership fee to track their CPD. In summary: The ESP will allow eligible SMIGs to work in Australia much faster. By removing the college assessments, you will save 10 – 12 weeks. At this stage, it seems clear that this will not be a cost saving endeavour for those pursuing Fellowship. I believe that this new pathway will be ideal for those at the early stages of relocating to Australia. If you are having trouble deciding, get in touch. Last Friday Shayla O. met with a husband-and-wife doctor team over coffee at Blacktown Hospital to help them plan their transition into general practice. I usually begin these meetings by asking what the doctor(s) hope to get from our conversation, and like many, this lovely team weren’t entirely sure where to begin. Thankfully, by the end of a great chat and some delicious coffee, they both felt a lot more confident and excited about the entire process. Meeting face to face gave us the chance to really talk through their goals and aspirations for their entire family. We were able to cover GP training programs, locations, grants, incentives, and talk about how all of these elements can influence each other. These in-person conversations always provide valuable insight, helping me better understand how to match candidates with practices that align with their goals and needs.
