** INDUSTRY SAFETY ALERT ** Summary: A continuous miner operator was struck on the head by a monorail trolley when it fell from the end of the rail in a development production area. There was no mechanical stop on the end of the monorail to prevent the trolley from falling. Recommendations to industry: The omission of a simple but necessary control can have serious consequences for workers. When extending monorails, it is essential that the monorail brake is secured at the end of the line to prevent the trolley from falling off. Operators should do a visual check to ensure the brake is in place before proceeding forward and engaging the trolley. ** INDUSTRY SAFETY ALERT ** Summary: Heating occurred on an underground conveyor from a failed roller. While undertaking conveyor inspections, a deputy smelt something burning and found 2 x 5 mm glowing embers under the belt. The roller bearings had catastrophically failed, allowing the roller shell to run on the shaft, creating excessive friction and heat. Recommendations to industry: Mine operators must have systems to identify and change-out defective conveyor rollers. Workers conducting conveyor inspections must be aware of the increased risk of roller failure at high tension areas of conveyors. They must also diligently inspect for fire risks such as accumulation of material, failing or collapsed idlers and contact between conveyor belts and fixed structures. ** INDUSTRY SAFETY ALERT ** Summary: A fitter was working on a roof support ram when a disconnected hose whipped around and hit him on the forearm, causing a laceration. The fitter was conducting planned maintenance at the time, troubleshooting why the roof support would not advance. The fitter isolated and dissipated the stored energy in the roof support correctly and had also checked the hose end was twisted before removal of the staple. It was found that the shuttle valve (which is piped to the rod end of the ram) underwent an internal malfunction that caused a pressure lock in the ram hose. The fitter could not have reasonably recognised this before removing the hose staple. The defective shuttle valve was the reason the support would not advance and was about 17 years old, (as old as the roof support). Recommendations to industry: When mining in an inferred area of geological disturbance strata, trigger action response plans (TARPs) are to be used as a proactive tool. That is, support should increase for both roof, and ribs. Cut-out distances should be adhered to for a higher-level TARP. ** INDUSTRY SAFETY ALERT ** Summary: A large rib fall of about 5 m long x 3.1 m high x 1.2 m deep occurred when cutting in a herringbone mains panel. No one was present on the side of the continuous miner where the fall occurred, although a worker was on the opposite side platform. It did not appear to be a lubricated fault (greasy back). Rib support consisted of 1.2 m bolts at about 2.2 - 2.4 m above the floor. Recommendations to industry: When mining in an inferred area of geological disturbance strata, trigger action response plans (TARPs) are to be used as a proactive tool. That is, support should increase for both roof, and ribs. Cut-out distances should be adhered to for a higher-level TARP. Earlier this year, we successfully completed a significant project for a major client, involving the design, engineering, and supply of Ventilation Control Devices (VCDs) for their Underground Booster Fan installation. This project presented us with critical constraints and tight tolerances, requiring careful design to accommodate the large fan components. We’re proud to share that the installation went off without a hitch—every component fit perfectly, with no modifications needed during installation or commissioning. The project was commissioned a few months ago, and the fan has been effectively ventilating the workings since then.
