We work closely with Ramsay Health Care hospitals to support the transition of individuals from hospital to home, providing seamless and integrated quality healthcare in the home and community setting. Ramsay Connect understands healthcare extends beyond the hospital and works with individuals to ensure health and wellbeing goals are addressed after leaving hospital. The first days and weeks following a hospital stay can be difficult. Ramsay Connect assists through this period, supporting individuals to thrive in their home as quickly as possible. The healthcare professionals providing the program are experienced in community care and committed to optimising each person's recovery.

    Last week we celebrated OT week! To celebrate, we had a chat to our very own senior team member with a background in OT Breanna Ellis (Pictured with Kellie Steer and Ian Galvin): The theme for this year’s National Occupational Therapy Week is “Celebrating 80 Years of Connections,” marking the 80th anniversary of the establishment of Australia’s first Occupational Therapy club. From early on, Occupational Therapy intrigued me. A family member once suggested I might be well-suited to the field, and I was fortunate to pursue my studies in OT at Charles Sturt University. Initially, as a student OT, I envisioned myself as either a hand therapist or a paediatric specialist. However—perhaps fortunately for patients!—I quickly discovered that making splints wasn’t my strong suit, and working with children didn’t resonate with me as I had expected. Since graduating 22 years ago, my career has been remarkably diverse. In my early years, I worked as a generalist OT across regional and remote Australia. Later, I transitioned from clinical practice into roles within government, non-government, and private health sectors, where I focused on strategic policy and leadership across areas like Aged and Disability Services, Chronic Disease, Cardiovascular Health, and Integrated Healthcare. Looking back on two decades in this field, I still proudly identify as an OT, even though my role is no longer clinical. At the heart of being an OT is the desire to connect with people, to understand how they live their lives. We think creatively, solve problems, and work toward meaningful outcomes—not just for ourselves but for those we serve. These skills extend beyond clinical work; they are essential in strategic health service delivery and leadership as well. I am extremely fortunate in my role with Ramsay Connect I can utilise my skills and knowledge that I developed from being an OT to work closely with my Ramsay colleagues and other stakeholders and challenge some of the health system norms and work on innovative ways we can ensure Australians can access healthcare at the right place and right time. And finally a massive shout out to all my OT colleagues if your still have your “creative occupational pieces” that you had to make for one of your subjects while in university let me know (extra points if you weaved a basket)!     This World Heart Day we want to highlight the importance of heart health. Cardiovascular disease a global health issue and a leading cause of death worldwide. It affects over 15% of the Australian population.   The Ramsay Connect Virtual Heart Failure Service supports people living with heart failure by empowering them to understand their condition and gain confidence in self-managing their symptoms. Our service includes access to physiotherapy and dietetic virtual consultations, and specialist nurses who remotely monitor patient vitals signs and provide individualised education. Better symptom management results in improved quality of life and reduces potential trips to the hospital.   With a Net Promotor Score of over 83, calculated from feedback from our past patients, we can confidently say patients feel real value in participating in our Virtual Heart Failure Service.     Congratulations to the Ramsay Connect Virtual Heart Failure Service team! Ramsay Connect has been awarded the Clincal Practice Prize at the Australian Cardiovascular Health and Rehabilitation Association (ACRA) 2024 Annual Scientific Meeting. Our abstract and presentation ‘Transforming heart failure care: virtual solutions improving patient access and outcomes’ was judged as the winner from a field of outstanding services being delivered and evaluated across Australia. Thank you to the ACRA ASM organising committee for hosting all delegates in what was an inspiring and engaging ASM.

    This diabetes week we celebrate the recent launch of our new pilot program, in collaboration with Australian Unity Aboriginal Home Health and The University of Queensland, Diabetes Using Our Strengths Service (DUOSS). DUOSS is a technology enabled service designed to be flexible in its delivery, allowing patients to receive care most suitable to their health needs. This includes service delivery in the home, in the community, by telephone or through remote patient monitoring.   The service will provide holistic care and support in collaboration with the patient's existing health care team. Participants who join the program will learn more about diabetes, how it can change their body and get culturally appropriate support to manage their condition. This includes appointments with a Diabetes Educator, Registered Nurse and Aboriginal Health Worker. Participants can decide what they want to achieve by setting their own diabetes health goals.   This project has been made possible through MRFF funding from the Australian Government's Targeted Translation Research Accelerator program, delivered by MTPConnect. Together, we are committed to making a meaningful impact on the health and wellbeing of Aboriginal and Torres Strait Islander people. #ramsayconnect #ramsayhealthcare

    This week marks NAIDOC week, a week to celebrate and recognise the history, culture and achievements of Aboriginal and Torres Strait Islander peoples. At Ramsay Connect we’re proud to have recently launched a new project titled Diabetes Using Our Strengths Service (DUOSS). Utilising Ramsay Connect’s experience and expertise in virtual integrated care, in partnership with Australian Unity Aboriginal Home Health, a series of yarning sessions and workshops took place on Bundjalung Country (Tweed Heads, Northern NSW) to develop and refine a culturally safe and appropriate model of care for Aboriginal and Torres Strait Islander people living with diabetes. These sessions took place with Elders with lived experience of diabetes, Australian Unity Aboriginal Home Health staff already supporting Elders with their aged care needs, and other local providers in the area to ensure the service integrates into existing services in the community. DUOSS leverages the existing strengths within the Bundjalung community. These include community and family connections, as well as social supports to improve the outcomes of those living with Type 2 Diabetes. With the model now refined, the service has been recently implemented and will be evaluated in collaboration with The University of Queensland.     This project received MRFF funding from the Australian Government’s Targeted Translation Research Accelerator program, delivered by MTPConnect.

    This heart week we recognise the important service our Ramsay Connect team provides via the Virtual Heart Failure service – an at home monitoring program designed to monitor the heart health of those with heart failure. The technology is used to support people with heart failure while they’re at home. Their vital signs, such as their heart rate, blood pressure and oxygen levels, are remotely monitored by a Ramsay Connect nurse to detect any changes early, and hopefully prevent the need to go to hospital. The program includes virtual consultations with specialist nurses and allied health professionals to help patients manage their medication, diet, exercise and mental health.  It’s a great example of how Ramsay is embracing technology for better patient care.     Today we honour and remember our founder, Paul Ramsay AO, who passed away exactly 10 years ago today. Paul was a philanthropist and iconic figure who helped shape Australia’s healthcare landscape for the better. So much has changed in 10 years, but one thing that remains the same is Ramsay Health Care’s commitment to people - and this is something that would make Paul proud. Paul lived by the phrase “people caring for people”. It’s a philosophy we continue to uphold today through looking after our patients, our staff and all our healthcare partners. As we celebrate Ramsay Health Care’s 60th anniversary in 2024, it’s a chance for us to reflect on Paul’s legacy and pledge to continue to lead in The Ramsay Way for many more years to come. #RamsayHealthCare #PeopleCaringForPeople #TheRamsayWay
