A Brisbane house sustained significant damage when liquid LPG escaped from a 45kg cylinder, forming a gas cloud that ignited. The fire caused extensive damage to the house, a timber fence and the neighbouring property. Initial investigations identified that the LPG cylinder had severe corrosion at the base, causing a pinhole leak. Remember to regularly check LPG cylinders for corrosion and damage. If you fill LPG cylinders, you must ensure they are free of defects before filling. Read our Safety Alert for key issues and recommendations. https://lnkd.in/gZzkiq7S

This animation recreates the events leading up to an incident involving a dozer that fell from its support stands. It’s one of three recent high potential incidents involving plant falling from support stands at Queensland coal mines. While no workers were injured, the risk for serious injury or even fatality was significant. For more information, read our safety bulletin. https://lnkd.in/gENqWGXK
