OKAN in Vestibular Diagnostics A study by Marie Reynders et al., published in Frontiers in Neurology (Feb 2024), highlights the diagnostic value of optokinetic after-nystagmus (OKAN) in vestibular disorders. The meta-analysis shows that OKAN time constant (TC) is significantly shorter in patients with vestibular issues compared to healthy individuals, suggesting it could be a useful tool for detecting vestibular deficits. New research on the Supine Roll Test (SRT) in horizontal canal BPPV reveals how initial debris position and test sequence affect nystagmus patterns and diagnosis. Thanks to Bhandari et al. (2022). (Front Neurol.) for this 3D simulation study which found that starting the SRT from the right or left side generates different nystagmus types—bilateral direction-changing, bilateral direction-fixed, or unilateral—depending on debris location. These findings suggest a standardised test protocol is needed for more accurate diagnoses. Be sure to check out the great videos at the end of the article. Minimum Stimulus Strategy for BPPV: This clinically relevant study by Libonati et al (and presented at Barany 2024) outlines the Minimum Stimulus Strategy for diagnosing BPPV. The Upright BPPV Protocol (UBP) is performed in seated position, using head movements along exact spatial axes to detect and treat BPPV, with video-Frenzel goggles monitoring nystagmus. The UBP: Focuses on minimizing patient discomfort. Increases diagnostic sensitivity through precise head movements. Uses nystagmus monitoring for accurate diagnosis of horizontal and posterior canal BPPV. Vitamin D and BPPV: Exploring the Link In this comprehensive literature review of recent research (2024), Rhim and Kim highlight the connection between vitamin D deficiency and the recurrence of Benign Paroxysmal Positional Vertigo (BPPV), and the efficacy of vitamin D supplementation in reducing the likelihood of relapses. Key findings: Vitamin D deficiency is identified as a risk factor for BPPV recurrence, alongside age, female sex, and other conditions like hypertension and diabetes. Vitamin D supplementation in patients with low levels significantly decreased the number of relapsing patients and relapses per patient. Important Insights on BPPV and Migraine! This recent study highlights how migraine can significantly worsen clinical presentation and recovery in patients with Benign Paroxysmal Positional Vertigo (BPPV). BPPV Patients with migraines showed more severe vestibular, dizziness and imbalance symptoms and had lower quality of life scores.
