Care-- the value of care.
Cormac Russell Nurturing an Ecology of Care It takes a village and an Enabling State. Here at Caregivers Alberta talking about CARE. Really we have to value relationships and care as much as we value cure. Put the Person at the Centre of CARE.

Decoding the persistence of delayed hospital discharge: An in‐depth scoping review and insights from two decades comes from the DeGroote School of Business - McMaster University Alyaa Abdelhalim Manaf Zargoush Norm Archer & Mehrdad Roham They did a review of systematic reviews over the last 24 years and their review highlights several factors of Delayed Hospital Discharge related to family caregivers: 1. Impact on Patient Well-Being: Caregivers often experience stress and pressure related to the discharge process, which can affect their well-being and the care they provide to patients. The anxiety and uncertainty surrounding discharge can lead to feelings of mistrust between caregivers and healthcare providers. 2. Need for Support and Training: The review suggests that providing timely information and support to family caregivers is crucial. Training for caregivers on post-discharge care, medication management, and recognizing warning signs of potential complications can help facilitate smoother transitions from hospital to home. 3. Variability in Definitions: The definition of caregivers varies across studies, with some referring to unpaid family or friends assisting patients, while others include healthcare and social care practitioners. This inconsistency highlights the need for standardized definitions to ensure accurate research and understanding of caregiver experiences. 4. Involvement in Discharge Planning: Engaging caregivers in the discharge planning process is essential. Their input can help tailor discharge plans to better meet the needs of patients and ensure that caregivers are adequately prepared for post-discharge responsibilities. Overall, addressing the needs and experiences of family caregivers is vital for improving discharge processes and reducing delays associated with DHD. https://bit.ly/3YtwHJw Notably, there are examples of way to improve hospital discharges-- The Ontario Caregiver Organization has 1. Essential Care Partner Practices for Hospitals https://lnkd.in/gp2_rwQ8 2. Essential Care Partner Program Implementation Toolkit: A Guide for Hospitals https://lnkd.in/gN3kgNti and in the US -- 40 states have enacted the AARP Policy, Research, and Thought Leadership Caregiver Advise, Record, Enable (CARE) Act The CARE Act requires hospitals to: 1. Record the name of the family caregiver on the medical record of your loved one. 2. Inform the family caregivers when their loved one is to be discharged. 3. Provide the family caregiver with education and instruction of the medical tasks he or she will need to perform for the patient at home. https://lnkd.in/gG9azKbS Barb MacLean Amy Coupal, M.
