    𝗗𝗼 𝘆𝗼𝘂 𝗸𝗻𝗼𝘄 𝗵𝗼𝘄 𝘆𝗼𝘂𝗿 𝘀𝗮𝗳𝗲𝘁𝘆 𝘀𝘆𝘀𝘁𝗲𝗺 𝗲𝗾𝘂𝗶𝗽𝗺𝗲𝗻𝘁 𝗰𝗮𝗻 𝗳𝗮𝗶𝗹? In functional safety, 𝘶𝘯𝘥𝘦𝘳𝘴𝘵𝘢𝘯𝘥𝘪𝘯𝘨 𝘵𝘩𝘦 𝘧𝘢𝘪𝘭𝘶𝘳𝘦 𝘮𝘰𝘥𝘦𝘴 of equipment is critical for ensuring system safety. Four key failure modes in safety systems equipment: 𝗗𝗮𝗻𝗴𝗲𝗿𝗼𝘂𝘀 𝗙𝗮𝗶𝗹𝘂𝗿𝗲: A failure mode that prevents the safety function from executing when a demand occurs, potentially leading to a hazard. This is the failure mode we need to control when designing for SIL. 𝗦𝗮𝗳𝗲 𝗙𝗮𝗶𝗹𝘂𝗿𝗲: A failure that causes the safety function to execute without a demand from the process. This leads to unnecessary shutdowns. But worse, shutdown and start ups are often the most dangerous modes of a process. A safe failure that is not so safe after all. 𝗡𝗼 𝗘𝗳𝗳𝗲𝗰𝘁 𝗙𝗮𝗶𝗹𝘂𝗿𝗲: A failure of a component that is part of the safety function but has no effect on the system's ability to perform its safety function. The safety function still operates as expected. 𝗡𝗼 𝗣𝗮𝗿𝘁 𝗙𝗮𝗶𝗹𝘂𝗿𝗲: A failure of a component that is not part of the safety function (but of the safety system) and therefore has no impact on the safety function’s performance. The system continues to work as intended despite the failure. 𝗗𝗲𝘁𝗲𝗰𝘁𝗲𝗱 𝘃𝘀. 𝗥𝗲𝘃𝗲𝗮𝗹𝗲𝗱 𝗙𝗮𝗶𝗹𝘂𝗿𝗲𝘀 The above failure modes can be detected or revealed. • 𝗗𝗲𝘁𝗲𝗰𝘁𝗲𝗱 𝗙𝗮𝗶𝗹𝘂𝗿𝗲𝘀: Failures discovered through built-in diagnostics. These diagnostics are designed to continuously monitor the system and trigger alerts and safe states when something goes wrong. • 𝗥𝗲𝘃𝗲𝗮𝗹𝗲𝗱 𝗙𝗮𝗶𝗹𝘂𝗿𝗲𝘀: Failures not detected via diagnostics can still be revealed. For example via proof testing. Proof testing does not detect failures in "real-time" but a good proof test ensures that undetected failures can still be found and acted upon. 𝗪𝗵𝘆 𝗶𝘀 𝘁𝗵𝗶𝘀 𝗶𝗺𝗽𝗼𝗿𝘁𝗮𝗻𝘁? For safety-critical industries, dangerous undetected failures are the most concerning because they can lead to catastrophic events without warning. Better to buy devices with excellent diagnostics then to wait for a proof test that might not take place for a year.
