A timely editorial in The Lancet, and a call to action. With the expansion of geographic footprint of dengue that places even more people at risk of this preventable disease, we cannot continue on this reactive response to dengue outbreak. Pre-emptive responses to prevent dengue outbreaks are vital. We even have new tools - Wolbachia, vaccines and new insecticides - to prevent dengue. Not tenable for us to ignore the tools we now have to prevent debilitating illness and save lives.

The timely laboratory diagnosis of Dengue infection is critical to inform both clinical management and an appropriate public health response. Frazer & Norton (2024) reviewed laboratory diagnostics in the dengue vaccination age. Based on the authors available diagnostic methods include viral culture, the serological detection of Dengue-specific antibodies in using enzyme immunoassays (EIAs), microsphere immunoassays, haemagglutination inhibition or in lateral flow point of care tests. They concluded that the impact of vaccination on immune response must be recognized as it will impact test interpretation and diagnostic algorithms. Reports of dengue have increased tenfold over the last twenty years. Nearly half of the world's population is now at risk. How has it spread so quickly? It’s complicated – involving everything from vaccine access to urbanisation and international travel. Of course, there’s another factor at play too: climate change. To explore the effects of climate change on malaria and 20 neglected tropical diseases (NTDs), and potential effect amelioration through mitigation and adaptation, Klepac at el. (2024) searched for papers published from January 2010 to October 2023. Of 511 papers meeting the inclusion criteria, 185 studied malaria, 181 dengue and chikungunya and 53 leishmaniasis; other NTDs were relatively understudied. Mitigation was considered in 174 papers (34%) and adaption strategies in 24 (5%). Roiz et al. (2024) investigated the global economic costs of invasive Aedes mosquitoes and Aedes-borne diseases. They concluded that the cumulative reported cost amounted to at least US$ 94.7 billion and despite costs are increasing only a modest proportion (1/10) is invested in prevention.
