Today’s cardiovascular case study is a PCI treatment of an elderly patient with previous stroke and renal insufficiency. The patient is a 78-year-old female who was admitted in the hospital with unstable angina pectoris having complained of intermittent chest tightness and pain for two years. The diagnosis confirms coronary heart disease. The patient suffers also from an old cerebral infarction, chronic gastritis, abnormal liver function and renal function, hypothyroidism, and type II diabetes. The patient’s CRUSADE score is 58, which indicates very high risk. Considering the patient’s age, moderate chronic kidney disease (eGFR 45ml/min), and a history of cerebral infarction for more than six months, the patient meets three minor criteria in the ARC-HBR criteria for high bleeding risk patients. Therefore BioFreedom™, a polymer-free drug-coated stent that requires only one-month DAPT therapy post-PCI, was chosen, and the result was satisfactory. BioFreedom™ offers a more flexible and less burdened clinical medication option for patients, especially for patients with high bleeding risk.
