First start be ensuring that your brand's core values, mission statement & goals, and value proposition are clearly defined. If one or more of those is no longer relevant, then you have to dig much deeper. How do you find out? Ask. Assuming those are still relevant, now focus on your brand image - and understand the path you are willing to go down. Are you prepared for a full "remodel" which could be immediately impactful, but costly? Or do you simply want to evolve the image over time and reinforce your relationship with your customers, but risk possible inconsistency along the way. In either case, take your customers on the journey with you, get their feedback, when possible, their buy in.

Start with understanding who your customers are -- and who isn't. Then its about understanding not just their product preferences but also their personal brand identities: their beliefs, lifestyles, what they stand for, how they live, and so on. Then it's about crafting an image aligned with these. In doing so I think it's about focus, and deciding who to be -- and not to be. All things to all people risks no things to everyone.
