Acetabular fractures in elderly remain to be a challenge. The assessment of the patient‘s risk and injury factors and the individualised treatment plan including the right choice of implant, are prerequisites for a positive outcome. 𝗣𝗮𝘁𝗶𝗲𝗻𝘁 𝗰𝗮𝘀𝗲: 85-year-old female patient, condition after minor trauma, complaints in the right hip and pain-related inability, osteoporosis and hypothyroidism. Dislocation of the cup through the fractured acetabulum and adjacent haematoma of the m. iliopsoas. Protrusio acetabuli, fracture extension into the posterior column (Paprosky-Type IIIb) and fractures in the area of the pubic bone (ramus superior and inferior ossis pubis). Intraoperative findings: pronounced bony defect combined with only moderate bone quality, fracture of the posterior column near the acetabulum on the ischium, complete destruction of the quadrilateral area. The supraacetabular dome was still present as a sufficient block. Treatment: Removal of the cup, implantation of 𝘁𝗵𝗲 𝗺𝗼𝗱𝘂𝗹𝗮𝗿 𝗿𝗲𝘃𝗶𝘀𝗶𝗼𝗻 𝗰𝘂𝗽 𝘀𝘆𝘀𝘁𝗲𝗺 𝗠𝗥𝗦-𝗧𝗜𝗧𝗔𝗡 𝗠𝗮𝘅𝗶𝗺𝘂𝗺 (Ø 56 mm) with anatomical strap and intramedullary peg anchorage (modular iliac peg 70 mm), screw fixation, 𝗮𝗱𝗱𝗶𝘁𝗶𝗼𝗻𝗮𝗹 𝗼𝘀𝘁𝗲𝗼𝘀𝘆𝗻𝘁𝗵𝗲𝘀𝗶𝘀 (𝗢𝗥𝗜𝗙). No complications, stable joint conditions, positive healing process. Postoperative pain-adapted full weight bearing with walking aid, geriatric-early rehabilitative treatment.
