In recent years, more knowledge about the preclinical development of Crohn's disease and ulcerative colitis has become available. In this MEDtalk, Jonas Rudbæk, PhD student at PREDICT at Aalborg University, Denmark, explains how systemic inflammatory changes exist in preclinical development many years before the diagnosis of IBD. The unidentified trigger may induce pathological changes, including dysregulated immune responses, dysbiosis, intestinal barrier dysfunction, and glycol changes. #IBD #Crohns #UlcerativeColitis

Dr. Abhinav Vasudevan from Eastern Health Box Hill Hospital in Melbourne reviewed therapeutic drug monitoring (TDM) for non-TNF biologics and small molecule treatments in IBD. Findings showed better responses with higher levels of vedolizumab, ustekinumab, and risankizumab, but insufficient data supports routine TDM for dose adjustments outside anti-TNF therapies. #Treatments #IBD #Crohns #Gastroenterology

In this MEDtalk, Maiara Brusco de Freitas from PREDICT, discusses the impact of childhood-onset IBD on patients’ nutritional status. Results show that impaired growth and nutritional status can be observed several years before diagnosis, particularly in Crohn’s disease cases. The findings emphasize the importance of regular nutritional screening and early intervention. #IBD #Crohns #UlcerativeColitis #ECCO

Mette Holst, Louise Bangsgaard Antonsen, Henrik Højgaard Rasmussen og Palle Bekker Jeppesen present: Chronic intestinal failure in adults often requires lifelong home parenteral support. This emphasizes the importance of early referral to intestinal failure specialist centers for effective disease management, improving patient quality of life, and alleviating economic burdens. #IntestinalFailure #PatientQuality #Support

Mette Holst, Louise Bangsgaard Antonsen, Henrik Højgaard Rasmussen and Palle Bekker Jeppesen present: The Danish Intestinal Failure Group, representing five Danish IF centers, conducted an audit to provide a foundation for future joint development work. The audit reveals similarities in patient ages across centers and variations in disease severity and practices, such as follow-up and ambulatory infusion pump use. #IntestinalFailure #Patients #Gastroenterology

Mette Holst, Louise Bangsgaard Antonsen, Henrik Højgaard Rasmussen and Palle Bekker Jeppesen present: Around 600 patients in Denmark rely on home parenteral support for survival. This qualitative study highlights key challenges to their quality of life, including dependency, fear of catheter infections from untrained staff, limited social life, and travel difficulties. Highly qualified staff in IF centers contribute to improved QOL. #IntestinalFailure #ParenteralSupport #QualityOfLife

How do patients with Crohn’s disease and perianal fistulas experience the impact of the disease on their sexuality, and how do they perceive the dialogue with healthcare professionals about this topic? This is what is explored in a recent study, presented in this MEDtalk by IBD nurse Karina Winther Andersen. #Crohns #Sexuality #ECCO24

In this MEDtalk, Thea Wiken elaborates on her study presented at ECCO 2024, comparing the effect of vedolizumab and ustekinumab in patients with ulcerative colitis who previously have been treated with anti-TNF-alpha inhibitors. The results show increased clinical remission in both groups, but only increased biochemical remission in patients treated with ustekinumab. The study also suggests that therapeutic drug management can have potential benefits in the treatment of ulcerative colitis patients. In this MEDtalk, Dr. Christian Selinger presents a systematic review that his research group conducted into reproductive safety issues of novel small molecules for the treatment of inflammatory bowel disease. The study aimed to describe the reproductive outcomes of the JAK inhibitors tofacitinib, filgotinib, and upadacitinib and the S1P inhibitor ozanimod regarding their effects on pregnancy outcomes. #IBD #Pregnancy #JAKInhibitors
