The solution starts with having a open discussion with team member. This discussion should not only cover work related stuff but also need to have a discussion around things happening at personal end. This journey needs to be start with empathy and later on build a personalise plan to bring team member on track. In some situation engagement of HR partner also helps for a guided journey. The most important stuff is that during guided journey, a close and careful observation is required week on week basis to ensure that team member get immediate feedback (Positive or Negative)

The very first questions, that will structure the behavioral answer should be : - what is that team member struggling with? - what is the underlying problem? Starting from that point, then, the real action could start as, as a manager, you'll be able to address a specific point. Don't forget that, the more active you'll be on the matter of that team member, the more positive message you'll send to the rest of the team.
