In my experience, many risks to a project can be identified during the discovery and requirements gathering stages. While working with project teams and stakeholders during current state process mapping and requirements gathering at the outset of a project, I provide the team with a list of areas for potential risk, like scope, budget, resources, etc. and brainstorm potential risks. Additionally, requirements typically include a list of assumptions. A rule of thumb is that for each assumption, you should list at least one associated risk. An easy question to ask is "If my assumption is wrong, what are the implications to the project?" Then add those risks to a risk register so you can assign ownership and proactively track their progress.

I make it a point to ask the team during meetings about potential challenges they foresee, which helps everyone start thinking ahead. I also implement regular risk reviews as part of our process. We revisit possible risks throughout the project, not just at the start, and adjust our plans as needed. To help with mitigation, I promote a solution-oriented mindset, encouraging the team to not just identify risks but also think of ways to address them early. This creates a culture where everyone feels responsible for managing risks, helping us avoid surprises later on.
