In a context of increasing diversity of RWD sources, including wearables and digital technologies that collect data passively, researchers need more discerning approaches for treating missing data than ones they’ve used in the past. The key to addressing uncertainties that arise from missing data is paying close attention to data collection methods and the intention behind why certain data are collected. This means recognising that some data are not collected by design but rather as an artefact of other, intentionally collected data. An example might be a remote monitoring device or app that collects mobility-related data (e.g., steps walked) but whose primary purpose is to monitor symptoms of dementia or atrial fibrillation. If a user incidentally turns off the mobility feature and data end up missing, this may not necessarily be cause for concern for researchers interested in the primary endpoint. The integration of RWD, AI, and other advanced technologies is transforming the landscape of clinical trials. One of the most notable innovations it has enabled is the development of hyper-precise natural disease histories, disease models, and trial simulators. This helps researchers and operators determine optimal values for trial execution before patient recruitment begins, therefore reducing cycle times and costs. But improving trial efficiency is not the only benefit of RWD and AI integration. As the DIA Global panel discussion summary (linked below) attests, in the patient-centric era we live in, it is also critically important to improve trial design so that protocols are more attuned to patients’ experiences, needs, and preferences. For that, sponsors need insight into the patient voice – for example, by understanding ePROs in the form of social media sentiment. Advanced technologies are crucial to capturing and analyzing such evidence at scale. RWD and RWE have many advantages for organisations that leverage them to their full potential, but also some disadvantages. One of the biggest drawbacks is the varying acceptance of RWE as “legit” evidence due to the role of unmeasured confounders. In the fall of 2022, PhRMA and BIO conducted a workshop on the subject of unmeasured confounders. A report that captures the main themes and learnings from the workshop has just been published and includes a discussion of novel methodologies for addressing the issue. One of the most interesting aspects of the report is the discussion around fit-for-purpose, research-grade RWD that needs to be intensively curated before being transformed into high-quality, trustworthy evidence generation (pages 11-12 in the report). Among its many uses and applications, RWD is critical for identifying optimal second-line and combination therapies when a first treatment for hypertension is not enough to control the disease. This is the thesis of a research project recently launched by the Yale School of Medicine and supported financially by the U.S. National Institutes of Health. Conducting RCTs for understanding the most effective secondary therapies or the precise combination of antihypertensive therapies that are most suitable for patients with different characteristics and to compare every possible drug and combination would be incredibly time-consuming and costly but gleaning such information retrospectively is more efficient with RWD collected while treating such patients. RWE-informed clinical guidelines that factor in the interplay between different secondary therapies and patient subgroup characteristics should be developed based on those insights, the project’s leader, Yuan Lu, says. While Yale is far away from the EU and from Cegedim Health Data’s network of data provider sites, what unites us is that the prestigious university’s research uses the OMOP Common Data Model, just as we do when curating and pre-processing the RWD we make available with THIN® databases.
