It is important to note that not all incidents reported by industry involve an injury; it could be a company notifying Health Canada of a non-compliant product or other issue. Consumers are encouraged to report health or safety concerns to help Health Canada monitor the safety of consumer products and cosmetics and to help prevent further incidents. US-FDA: Guidance Document, Voluntary Malfunction Summary Reporting (VMSR) Program for Manufacturers (August 2024) The FDA has issued this guidance to assist manufacturers in better understanding and utilizing the Voluntary Summary Malfunction Reporting (VMSR) Program. This program, established by the FDA, allows manufacturers to submit certain device malfunctions with specific product codes in a summary format on a quarterly basis. The VMSR Program is designed to offer several advantages, including increased transparency for the public, more efficient processing of malfunction reports by the FDA, easier identification of malfunction trends by both the FDA and the public, and reduced reporting burdens for manufacturers. Two new quality standards, one for MDMA and another for psilocybine, have been published. These standards each specify the minimum quality requirements for both the active pharmaceutical ingredient (API) and finished product.
