#Hepatic_macrophage populations exhibit dynamic changes throughout the progression and resolution of #liver_fibrosis. During fibrogenesis, macrophages are recruited to the liver and polarize into pro-inflammatory phenotypes, which exacerbate #fibrosis by releasing #inflammatory cytokines and activating #hepatic_stellate_cells (HSCs). Conversely, in the fibrotic resolution phase, the tissue-resident macrophages peak and acquire an anti-inflammatory M2 phenotype, promoting #HSCs apoptosis and extracellular matrix degradation. This transition in macrophage phenotype is driven by the phagocytosis of cellular debris and the secretion of anti-inflammatory cytokines in the surrounding #microenvironment. #MSCs are key players in #liver repair and fibrosis regression by secreting various bioactive molecules that effectively induce the polarity of #macrophages towards an anti-inflammatory phenotype. This study assessed the impact of human #MSC-derived #extracellular_vesicles (EVs) on macrophages derived from UCB-monocytes. The results indicated that #EVs from Wharton’s jelly MSCs can induce activated macrophages to suppress immune responses and potentially play a protective role in the pathogenesis of #liver_fibrosis by directly inhibiting the activation of #HSCs. The dysregulation of the master regulator #c_MYC (MYC) is crucial in #hepatocellular_carcinoma (HCC), yet it remains a challenging target for treatment. #MYC expression is influenced by epigenetic modifications within naturally occurring DNA loop structures known as Insulated Genomic Domains (IGDs). This study proposes a therapeutic strategy involving an epigenomic controller (EC), a programmable #epigenomic #mRNA medicine, to alter MYC IGD sub-elements precisely. This results in the methylation of #MYC regulatory elements and a sustained reduction in MYC mRNA transcription. Notable antitumor effects have been observed in preclinical #HCC models treated with the MYC-targeted EC, either as a standalone treatment or combined with tyrosine kinase or immune checkpoint inhibitors. These results open avenues for the clinical development of #MYC-targeting epigenomic controllers for HCC patients and establish a framework for using programmable #epigenomic #mRNA #therapeutics in #cancer and other diseases. #Hepatocellular_carcinoma (HCC) is the third leading cause of cancer-related deaths globally. #HCC in many cases is a consequence of prolonged #liver_fibrosis, resulting in the formation of an intricate premalignant microenvironment. The accumulation of #extracellular_matrix (#ECM) is a hallmark of premalignant microenvironment. This study used liver micro-tissue as a #3D biomimetic #platform to evaluate the impact of fibrotic matrix. The results showed that the fibrotic matrix increased #epithelial_to_mesenchymal_transition in the micro-tissues compared to the normal matrix. Moreover, the data showed that #fibrotic matrix could induce de-differentiation in these 3D models. Our findings suggest a regulatory role for the #fibrotic matrix in promoting #cancerous_phenotype, which could accelerate the progression of malignancy in the #liver. ✔ Targeting Apolipoprotein to alleviate liver fibrosis: #Liver fibrosis, a pathological hallmark of chronic #liver diseases, has long posed a challenge in therapeutic management. In a recent study, researchers have identified a natural small molecule, 12-deoxy phorbol 13-palmitate that significantly mitigates #liver_fibrosis by targeting #apolipoprotein L2 (APOL2), a protein newly implicated in fibrosis progression. #APOL2 is activated by transforming the growth factor-β1 (#TGF_β1) pathway. This activation leads to the binding of APOL2 to sarcoplasmic/ER calcium ATPase 2 (SERCA2), which induces endoplasmic reticulum (ER) stress and enhances the PERK–HES1 signaling pathway activity, contributing to the progression of liver fibrosis. The study demonstrated that 12-deoxy phorbol 13-palmitate effectively downregulates APOL2 expression, inhibiting the activation of #hepatic_stellate_cells (#HSCs)—a key driver of fibrotic scarring.  In both in vitro and in vivo models, treatment with this molecule resulted in a marked reduction in #collagen deposition and liver stiffness, two critical indicators of fibrosis severity. In preclinical animal models, this compound improved liver function and halted disease progression, showcasing its potential as a therapeutic candidate. Its safety profile is especially noteworthy; as a naturally occurring compound, it exhibited minimal toxicity, making it a promising alternative to current antifibrotic therapies, which often have significant side effects. Further clinical studies are now anticipated to assess the molecule’s efficacy and long-term safety in humans. Despite significant advancements in therapeutic modalities for #hepatocellular_carcinoma (#HCC), there is still a high annual mortality rate with a rising incidence. Major challenges in #HCC clinical management are related to the development of therapy #resistance, and evasion of tumor cell apoptosis which leading unsatisfactory outcomes in HCC patients. Previous investigations have shown that #autophagy plays a crucial role in contributing to drug resistance development in #HCC. Evidence showed that #miR_29a serves as a negative regulator of #autophagy in other cancers. The current study showed that #EV20K_miR_29a treatment significantly induces cell apoptosis and reduces both cell proliferation and colony formation in HCC cell lines. In addition, the #LC3-II/LC3-I ratio was increased while the expression of key autophagy regulators #TFEB and #ATG9A were downregulated by this treatment. These findings suggest an effective blockade of #autophagy by #EV20K_miR_29a leading to #apoptosis through concomitant targeting of critical mediators within each pathway.
