The management of patients with #COPD and #comorbidities is particularly complex and involves several specialists as well as close collaboration with local healthcare facilities and general practitioners (GP).   The RE-SAMPLE project aims to involve not only the hospital but also the community through the establishment of Shared Care Facilities (SCF).  Indeed, collaboration between primary and tertiary care is essential for the efficient management of COPD symptoms and chronic diseases.   In Italy, Fondazione Policlinico Universitario Agostino Gemelli IRCCS has developed a dedicated dashboard where the GP can easily screen patients and share the result with the hospital health team to be included in the RE-SAMPLE virtual companionship programme. By implementing this new technology, the hospital and the GP can work together to enroll and manage the patient, making the care pathway more efficient. #community #collaboration #ehealth #technology #patient #carepathway #clinician

We are proud of our PhD student Leonardo Nucciarelli's participation at the IEEE #ICHI2024 Conference where he presented the Gemelli Generator - #RealWorldData (GEN-RWD) #Sandbox, a secure, scalable and flexible architecture for distributed analytics in #healthcare. Leonardo's presentation also showcased the Hospital Data Empowerment initiative, which is designed to empower external researchers in the use of hospital data in a privacy-by-design perspective, while preserving data ownership. During the two days, Fondazione Policlinico Universitario Agostino Gemelli IRCCS representatives Stefano Patarnello, Alice Luraschi, Agni Delvinioti, Giulio Pagliari, @Alessia Martini, and @Giuseppe torres had the chance to be again in contact with the consortium partners, and discuss about the upcoming #virtualcompanionship programme, #virtualcoaching as well as tailored tools that will be put into practice in a few weeks.
