Integrating the use of nasal high flow (NHF) across the ward (floor), emergency department (ED), and pediatric intensive care units (PICU) may be associated with improved standardization of care. When used cohesively across the hospital, NHF can also contribute to a change in respiratory support practice, moving towards less invasive strategies and the potential for more patients to be managed in local hospitals and lower acuity settings. Neonatal CPAP therapy can improve the work of breathing by reducing the energy required to expand the lungs. Work of breathing is the force required to expand the lungs against their elastic properties. CPAP has been shown to elevate end-expiratory lung volume through a continuous distending pressure, which helps to unload the inspiratory muscles and reduce work of breathing. The airways of neonates and infants are fundamentally different from those of adults in terms of anatomy and physiology. The delivery of respiratory gases to these populations requires special considerations for several reasons: To ensure maintained function of the mucociliary transport system, which plays a crucial role in airway defense; to aid in thermoregulation; and to ensure effective ventilation is provided to these inherently vulnerable patients.
