Our team made the long journey from our Newcastle office to the #IntegratedCareDeliveryForum2024 in London. This event focuses on connecting ICS leaders with innovators and industry experts, aiding local systems who are grappling with workforce and service challenges. This forum gives an opportunity for system leaders to discover best practice and discuss new transformation models. David Hambleton, Managing Director of Streamliners UK, appeared as a panel member exploring the question of how we can “Use data to deliver proactive population health”. Improving population health is dependent on a wide range of agencies, partners, and technology. The #HealthPathways approach to granular data collection yields meaningful insights into the reasons for variation in care. This in turn supports planning and priority assessments on local and national levels. About us
The HealthPathways Community is an international collective of health and social care organisations that collaborate to deliver locally relevant pathways to clinicians. Community members use the HealthPathways platform which includes starter pathways, pathway development tools, and websites for front line clinicians to refer to at the point of care. This collaboration delivers better patient outcomes by reducing variation in pathways and clinical practice and enabling new health service policy and clinical evidence to flow swiftly into practice. The HealthPathways development and review process asks questions that help reveal unconscious bias and assumptions about patient behaviour and service availability that the pathways can then address. HealthPathways accommodates necessary variation in care processes due to important local differences, while also helping to reveal unnecessary variation in care, which contributes to equity of outcomes.

HealthPathways is an important tool in enhancing inclusion and cultural responsiveness in healthcare. In New Zealand, we’re doing this is through integrating Māori language into pathways to make resources more accessible and #normalise te reo in clinical settings across Aotearoa. We’re proud to have released a clear standard to support our New Zealand teams to use te reo appropriately in their #HealthPathways content. Creating this standard was a thoughtful process. We drew on national standards and language principles recommended by Te Taura Whiri i te Reo Māori to develop a custom approach for HealthPathways that serves all clinicians across New Zealand. It accounts carefully for context to offer best practice for using te reo and translation guidance for key terms, providing an extra level of support to clinicians who are new to the country. Our team delivered four presentations and a workshop at the North American Conference on Integrated Care in Calgary last week. Themes of discussion at the conference included: 💡 building trust 💡 fostering collaboration 💡 scaling innovation 💡 learning health systems There was considerable interest in #HealthPathways as a practical enabler for all of these things, and a recognition of the value of being part of a unique global collaborative community, focused on making a difference. The team made some great connections and had the privilege of learning from global experts on integrated care. Thank you to the organizers for putting together an outstanding event! Yesterday our Co-Founder and Managing Director Sam attended the National Strategic Clinical Network for Musculoskeletal Conditions - Conference and network launch in Swansea The event launched the clinical Network and the "framework for arthritis and MSK conditions" and provided a day of interactive learning focussed on working in partnership to achieve our common goals. These things do happen...After some minor hiccups with our banner, (deftly handled by Dr Jon Tose, Clinical Lead, Streamliners UK and Lily Mirfin, Pathway Sharing Facilitator, Streamliners UK), our team enjoyed an excellent day at the Convenzis Group 8th NHS Integrating Care Conference: Building Interoperable NHS Systems. David Hambleton, Managing Director, Streamliners UK, chaired a round table discussion focussing on the operational pressures within the NHS such as addressing waiting times and managing financial constraints. Following the Darzi review, emphasis is placed on how to make the 3 shifts, from treatment to prevention, analogue to digital and hospital to community. These shorthand statements hide a complex mix of challenges, many of which underpin significant culture and behaviour change programmes. This session was centred around: ✅ How can we support GPs to manage their patients without needing to refer. ✅ How can primary and secondary care work together to agree care pathways. ✅ How those pathways can be standardised and produced at scale. ✅ What is the impact of standardised pathways on referral rates and waiting lists.
