About us

Learning Teams from Dr Todd Conklin, PhD, are part of a way of looking at safety, quality and operational excellence differently by a facilitated approach to worker engagement and supporting the empowerment of people to own safety, quality or operational excellence. A Learning Team is notable because it encourages organizations to obtain and consider different perspectives and angles of functional diversity to define a problem in a group context. The different perspectives that emerge from a Learning Team group demonstrate that no one person holds all the knowledge needed to solve complex problems. A Learning Team involves facilitated engagement (using a facilitator) with workers to understand and then learn from the opportunities that are presented by: 1) Everyday successful and safe work (Everyday Learning Teams 2) Events or incidents that could have or did harm workers (Event Learning Teams) 3) Introduction of changes (Management of change) that could affect worker safety (Periodic Learning Teams). Learning Teams support both worker learning and organizational learning by allowing the different stakeholders groups to understand better what, when, how, and why, people do things differently rather than following formal, written procedures or systems. By understanding what is necessary to make sure things go right, it is possible to focus on ensuring that factors which make things go right are present in the workplace every day.

    Due to popular demand, we have extended the 1 day workshop into the beautiful Blenheim region of Marlborough in the South Island of New Zealand to be held on Wednesday 27th November. Big thanks to Alistair Thomson and Anna McElhinney at Hortus Ltd. for supporting this event. Our new program “Unlocking Safer Machinery Use: Tackling the risk of machinery and designing betterwork” is for both operational, quality, environmental and safety people. It is a blend of face-to-face workshopping and group coaching sessions over a 3 month period. Bringing a HOP and learning lens to this risk. The workshop and coaching sessions cover: - Worker engagement and storytelling to identify machinery risks. - Risk management strategies that foster “Safe Work” and “Safe People” approaches in the use of machinery (ISO 12100/AS/NZS 4024.1201). - The role of human error, avoiding blame, valuing learning, understanding context, and responding effectively. - Learning methods for guarding against hazards, including engineering and administrative controls. Applying the hierarchy of guarding types for work setup, operations, clearing jams, cleaning, and maintenance. - Practices to identify operational waste and safety improvements from everyday work. These learning intentions are designed to ensure participants leave with both practical knowledge and an action-oriented approach to machinery risk. Participants will also have access to our Machinery Safety AI Tools , which; 1. Identify issues with processes and procedures to support why work goes well. 2. Making sense of the isolation and engineering elements of safeguards and safeguarding.     3 years ago today, Jeffery Lyth, Brent Robinson, Todd Conklin and I published a book called "Learning from Everyday Work: New View of Safety Discussion White-paper", ISBN 979-8487526328979. In that book, we explore gaining insights from the frontline. In one chapter of the book we said; "MAKING SENSE OF THE WORKER ENGAGEMENTS: We are capturing from the workers how they make sense of “the worker lens” of everyday work variability as business intelligence and insights, and presenting those activities and information in a visual representation of data “infographics dashboard” called an Operational Learning Dashboard, as a series of informational and actionable metrics. The organization then needs to be able to make sense of that narrative-based data by looking for trends, patterns of occurrence, of weak signals and the sentiment of the language used to identify weak signals. The correlation between the weak signals and the positive, neutral or negative sentiment of the narrative use by the workers provides additional insight to the “psychological safety” of the work team as they express their current resilience in the system they operate in through dialogue and group learning. If we were to think of that infographic as Jenga, we start by collecting the parts of the Jenga that have been removed and looking at the Jenga tower from all sides, even though the Jenga tower is still standing. We sort and group that information in a way that allows the information to be seen. We then present that information of trends, patterns and sentiment in a way that allows the organization to be curious and look at the underlying context to make sense of the bigger picture and take action by conducting a targeted Learning Team to learn and improve the system." We also introduced the community to the 4Ds, and since then, the 4Ds have been deployed to more than 3 million workers (that we know of). For the last 3 years, we have been deploying our 4D Work Insight App into various industries to explore and learn. Our 4D app worked by listening to the story from the frontline workers, taking "What they said" and identifying the weak signals, and then taking "How they said it" and painting those weak signals as colors of emotional sentiment (how people were coping in that work). Now, with the wider acceptance of AI tools like Microsoft Co-Pilot Studio and ChatGPT we are officially launching our offering to the market in the new year. The picture below is a teaser of our visualization approach to the 4Ds, and our "Digital Literacy" friendly app. We want this to be a sustainable solution for organizations to adopt. We will not be licensing based on the number of workers, sites, or the organization's size. Our offering will support small and large enterprises based on the number of learnings you want to gain insights from. Both quarterly and annual plans will be available as a managed service.     I recently finished reading Todd Conklin’s book “Workplace Fatalities: Failure to Predict” and loved the quote, “We can not manage uncertainty by managing probability and prevention. We must manage uncertainty by management of capacity…”. Inspired by this thought, I purchased “Controls Save Lives - The Sixth HOP Principle?” recently released by Brent Sutton and Jeffery Lyth. Less than 70 pages from cover to cover and it should be required reading for any leader seeking to address Critical Risks in a meaningful way while also building trust, learning and collaboration with their workforce. I have a meeting next week with almost every executive within our company and I just purchased 100 copies so everyone walks away with a book of their own👍🏼 I wish I could communicate these concepts as clearly and concisely as the authors - I have never felt more comfortable with the vision and direction of our organization’s safety strategy. #Cintra #ASAR #HOP #CriticalControls
