Send your resume to: carla@virtualbusinessstaffing.com or carmina@virtualbusinessstaffing.com Our website: https://lnkd.in/eM3PhcPQ Chronic Care Management Coordinator Job Responsibilities: • Validates patient consent for Chronic Care Management (CCM)/Remote Patient Monitoring (RPM) enrollment • Delivers non-face-to-face CCM for patients with two or more chronic conditions, meeting Medicare’s 20-minute monthly requirement • Supports patients, families, and caregivers via the inbound call system • Documents services and communications in authorized clinical systems, including internal and external EMR systems • Implements provider-driven, patient-centric care plans • Assists patients in goal-setting for self-management, health promotion, and hospitalization prevention under provider/QHP supervision • Executes CCM/RPM care plans under provider/QHP supervision • Completes quality documentation in structured, evidence-based templates • Collaborates with provider practices and partners for comprehensive information exchange • Educates patients on chronic disease self-management as per the care plan template • Gathers and documents ongoing patient information related to short-term and long-term goals in authorized clinical systems • Collects health data, monitors biometric alerts, and identifies care gaps such as RPM device non-adherence • Provides condition/disease-specific education within scope of practice under provider/QHP or RN guidance • Establishes patient rapport using motivational interviewing during CCM/RPM service period • Documents patient clinical diagnoses, behavioral health, and special needs data in relevant systems • Records data on potential physical limitations and social determinants of health in care plan • Delivers culturally competent care for improved care plan adherence • Facilitates healthcare service coordination and referrals under physician/QHP direction • Assists in gathering post-discharge patient utilization data and performs call outreach as needed • Implements care plan changes per provider, QHP, or RN directives • Promotes medication adherence • Coordinates appointments with providers/QHPs and vendors, including TCM, AWV, and RPM services • Updates patient medication list monthly and after each utilization event • Encourages adherence to PCP and specialist appointments • Guides patients in navigating their healthcare system • Involves patient caregivers and representatives in compliance with federal, state, and HIPAA regulations

Job Qualifications: • Must have a Bachelor of Science in Nursing, BSN (certification required) • Proficiency in both English and Spanish, with excellent verbal and written communication skills.
