Don't be bothered. Likely you have an issue with the install. So long as everything works why worry.

Well many ppl get some MENU settings reset after Updates (I think you mean, if a Version Upgrade e.g. STANDARD to DELUXE/PREMIUM then the versions may get reset also as they have different WASM access to Content).
Many had their Game bricked altogether by the XBOX ACCOUNT DOWNTIME. That is just beyond ASOBO and MS just do not give a crap to step in and make a fix for the next time it happens.
