Which one you pick and which DLC you get really does depend on which armies you like. I prefer the ancients because Rome. Also I like infantry heavy army and medieval seems more knights and cavalry.

The thing is I have a million games by now, and I keep bouncing around too much. I want to focus on a couple of games at a time and finish/master them so I kind of need to plan a bit otherwise I put a few hours in here and there but never play anything "seriously".
- the unit size is tweaked depending on scenario for historical battles in FOG II, and in Medieval there is also a modifier that scales unit size for custom games depending on the involved armies. Both games contain some historical huge battles and you can create either tiny or giant custom battles in both (for some armies in Medieval the unit size will scale, but this will not affect the number of units)
