Some of my couriers have more lead than brains at this point. I just keep repalying their adventures in the Mojave.
Most enemies don't move too much, so they arent hard to aim at in real time. Make small mouse movements and stay on the target.
Only things I find difficult to aim at are cazadors because they keep twitching around.
If you really struggle against everything, you can still use Vats or focus a lot on melee (it's not always easy to avoid damage in real time if you attack at melee range though). Also use turbo or implant GRX to slow time.
