If you mean the top left equipment item as in the Yoru saved equipment room, it's crafted by combining Duskwood, Hudan Leather, Shaedrite. If you mean the 2nd page of items, it's a talking door in Duty's Point, I believe.

Neither of those, actually - I mean the top left item when you access your equipment / talk to Uros at camp. This would be on the first page of the inventory screen, first (leftmost) item on the second row.

Oh, that one. You get that from the Library level. If you use the warp statue to get to the mid-point in the level and use the exit directly to your right (that used to be blocked by a shortcut switch), take the portal at the south of that room and use the southern entrance/exit and do that set of puzzle rooms, it's in there.

Ah, thank you, have it now. Solved the rooms but didn't grab the chest... and after a certain point, I've been assuming closed chests are just mimics and thus hadn't even tried to open it. :P Appreciate the help!
