For "no death", the tricky parts for me were The Gutter (gravity), Undead Crypt+Velstadt and the Throne Duo.
Iron Keep and Shrine of Amana might seem tough for that kind of run but if you just use a bow + lightning arrows and take it slow, it's pretty easy. And if you go for The Rotten-farm route, you wouldn't even have to do Iron Keep (or any other place with a Great Soul boss, except Gutter/Black Gulch). My bane was No Man's Wharf though, because i just kept wanting to run past everything after a failed attempt.
I eventually opted not to summon, but the OP summons sprinkled across the game can be a huge life-saver. DS2 NPCs are built different.
(Though the same is true for invading NPCs - the random, nonscripted Forlorn seem to be always aware when you're doing no death/no bonfire runs lol)
Every time i used a primal bonfire (you are allowed to use those to port back to Majula for no bonfire), i went to Dyna & Tilo and attacked the nest, to very quickly drain durability left on my weapons and then had Lenigrast repair them to full, since he will only repair fully broken stuff.
The true final boss are the credits. If you skip them, you will fail the conditions for both no death and no bonfire and not get the rings. The moment you last hit Nashandra (or Aldia if you went for the alt ending), put that controller away.
