Baba Yaga: documents achievement bugged?

is this achievement bug or is just me? how can I fix it?

Is every witchcraft document located in the haunted vale??

According to my 20/21 only missing document from Rise of the Tomb Raider (RotTR) to get an 100% Wicked Valley's documents colected "Achievment" they are all inside it.

One requires the Climbing Axes' Wiring Tool skill to get and the other is located upon a spot that requires a really tight timing to get it, which is hte one that I'm still missing.
