How to get "The Way of the Alchemist" achievement?

It says "Solve every puzzle in the main campaign". Here is the screenshot from the Appendix:

I have solved every puzzle from each of the 5 chapters, plus Prologue and Appendix, but I didn't get the achievement - any ideas what triggers it?

I have played the game in 2018, now I have only done two remaining puzzles in the Appendix.

I haven't done anything from the Journal in the bottom right corner.

I have found the answer...

In chapter five, the last item on the list is Concealed Letter - after opening it, the credits are displayed, without any option to skip them. Achievement is awarded after letting them play till the end.
