Lock your monitor refresh rate to 60. Sounds like some hardware configuration (or mod?) is trying to push the game over 60 fps, and that only results in what you are experiencing.

Thanks but i tried that. I've changed my monitor resolution and refresh rate. From 1440p/144hz to 1080p/60hz and still the same issue. I have no clue what i'm doing wrong :(

My son had the same problem with a game laptop, it took him a solid day to figure out how to ensure the laptop monitor was doing what he set it at. It eventually warned him he was violating the warranty etc... So I still think it is likely the monitor, but ¯\(°_o)/¯

Sooo... I changed my monitor to another one with 1080p/60hz at default and... Same issue. I tried to turn off gpu acceleration/windows game mode and everything else what is supposed to push my fps higher. I even changed destination of the game from SSD to HDD.

I dont know what's going on. My friend with better setup have no problems with this game.

I assume you already forced your GPU and verified game cache.

If not verify game cache first.

Make surve onedrive is not backing up DS constant auto saves to the cloud.

For some even bluetooth plugged in can create problems in some games so best to unplug everything not used.
Quit every third party programm not used.

Monitor settings>graphical settings>search for the game.exe and select high perfomace(RTX 3060).

Game is not demanding also you do not have the mouse icon not sure if not Steam version Dark souls 1 remaster is different-

Family share 😅 That's the same game but i don't own it on my own acc.

The game works perfectly on my gf computer (gtx 1050ti/i5-7400) and is also downloaded via family share.

Just clarifying, the "menu/gameplay" is not broken, this is a 'your' system specific problem, as you clearly post. Good luck with your search for solution, tho am still thinking it is monitor refresh related...

Did you read the thread? Are you trying to run the game above 60 fps? Lock monitor refresh rate to 60.

Otherwise your system specs? Don't make people guess if you are asking for help.
