No you don't need to restart it. You can enable them at any time in any playthrough, although I advise you to play the DLCs through a fresh start. Since you'd be too OP at this point in time to have any difficulty.

Quick question, if I was to buy the dlc's would I have to restart the game? because im on my first playthrough and im about to start trying the darkest dungeon quests now with around 9 level 6 heroes and I don't really wanna restart the whole thing

Indeed, they are Crimson Court DLC trinkets. Very useful for some fights and areas.

What are the trinkets on highway man, never seen them, are they dlc or something?
