Oliver Heimes will join the board. None of this is possible without our partners across the European healthcare system and the patients we serve. Doccla was founded to provide hospital-level care and support at home for multiple patient profiles across the acuity spectrum: enabling the early discharge of patients, as well as ensuring that for those with chronic care needs, hospital admission might be avoided in the first place. The funding will be used to further consolidate our market-leading position in the UK and Ireland (where we recently launched with the Health Service Executive), while looking to expand in France and the DACH region, and build upon our Pharma work. 'The Hospital in your Home' Yesterday I spent 40 mins with a patient and her daughter developing a Proactive Care Plan which included Advanced Directives and a RESPECT Plan (Recommended Summary Plan for Emergency Care and Treatment). It's never an easy conversation but it was a good conversation. We examined her wishes developing a personalised care and support plan according to the principals of the Personalised Care Institute. We discussed escalation of care and when she spoke about the care that she and her husband (recently deceased) had received over recent years, she said the care that if she were to fall ill again she would like care from the #UrgentCommunityResponse team and #HospitalatHome team if she was to deteriorate. Berkshire Healthcare NHS Foundation Trust This brought an internal smile to my heart ❤️ During the Pandemic whilst working in my role as NCD for older People NHS England I offered Clinical support for a new Policy to roll out 24,000 beds as part of a #VirtualWard Programme building on evidence of a previous NIHR study 'Is Comprehensive Geriatric Assessment Admission Avoidance Hospital at Home an Alternative to Hospital Admission for Older Persons? A Randomized Trial' (April 2021Annals of Internal Medicine 174(7)DOI:10.7326/M20-5688). We started the task of modelling with the brilliant Andrew Hoyal led by Stephanie Somerville and with Board support from Ian Dodge through Matthew Winn . A small team with Jane Sproat Rob Coster and David Russell gave us the opportunity to take this to Board and to then begin the work of rolling out the ambition for 24,000 beds across England (40 - 50 beds per 100,000 population) publishing the guidance in December 2021 and then starting the roll out in April 2022. The work wouldn't have been possible without an expert in the field someone to keep policy makers grounded and who to this day is supporting the Programme - Dr Shelagh O'Riordan a Community Geriatrican now leading a Getting It Right First Time (GIRFT) team who support the roll out. In collaboration with East and North Hertfordshire NHS Trust and Hertfordshire Community NHS Trust and Doccla, the Managing Heart Failure at Home (MHF@Home) pilot is proving the value of Remote Patient Monitoring (RPM) for supporting better outcomes and easing pressures on NHS resources. ✨ Key Outcomes from Our Pilot: • 100% Reduction in 30-Day Readmissions: RPM led to zero readmissions within 30 days, stabilising patients and conserving hospital resources. • 32% Drop in A&E Visits: RPM helped reduce emergency visits, empowering patients to manage their conditions from home. • £558,601 in Potential Six-Month Savings: Projected cost savings across ENHT’s heart failure patients demonstrate RPM’s impact on NHS budgets. We're thrilled to announce that Doccla's managing heart failure at home pilot programme has won the 'Most Promising Pilot' award at the HTN - Health Tech News Awards 2024! This award-winning pilot, conducted in partnership with Hertfordshire Community NHS Trust East and North Hertfordshire NHS Trust, showcased a proactive model of care for heart failure. By providing remote monitoring, guided self-management, and patient education, we successfully reduced readmissions to zero and significantly improved patient well-being. Key achievements of the pilot include: 🏆 Reduced 30-day readmissions from three to zero among 52 patients in just three months, saving the NHS approximately £6,822. 💸 Estimated potential savings of £558,601 over six months for the Trust's 4,176 registered heart failure patients. 🏥 Reduced A&E contacts and non-elective inpatient contacts. 🚀 Improved patient mobility, self-care, quality of life, and reduced anxiety and depression. This recognition highlights the potential of remote patient monitoring to transform healthcare into a more proactive, patient-centred model. Found Vin Diwakar’s answers to questions very interesting - when asked by Gillian Leng CBE how optimistic he was about digital health in the NHS in the future he said that when there is alignment between politicians, professionals - by which he meant everyone in the NHS - and the public, that’s where the magic happens. In response to tech that was making a difference he highlighted #VirtualWards He also flagged in his strong keynote that the reason the NHS is facing increased demand is not more people needing care, it’s the tripling of people living with multiple long term conditions - hence the missions from hospital to home, analogue to digital, sickness to prevention.
