We are looking for an enthusiastic and motivated Community Engagement Network Lead to work within our integrated Healthy Lifestyle Service, as an integral part of our Staffordshire service. The ideal candidate will be required to work in the North of Staffordshire and Moorlands Area, and will need to have a driving licence, access to a car or reliably commute to travel and support teams throughout the county as required. This role will be responsible for empowering the community to access our service through making and maintaining strong community links, helping other agencies understand the role of the service and through community outreach and engagement activities. If you are an excellent communicator who is enthusiastic, motivated, and looking to make a difference to local communities, then this is the role for you. The post-holder will: • Manage a small team of practitioners • Work to participate in all community networks of relevance developing trusted partnerships with relevant stakeholders to increase referrals into all services • Organise and facilitate promotional events • Deliver Making Every Contact Count (MECC), raising awareness of the service and ensuring the service can reach the residents who might benefit from it • Deliver MECC Train the Trainer sessions to enable others to deliver MECC training • Engage with existing organisations across the public, private and charitable sectors to embed the service Sounds like the role for you? An exciting opportunity is available for an experienced, enthusiastic, dynamic and motivated Physical Activity Specialist with a passion and keen interest in weight management! This role will contribute to the development, provision and coordination of the Adult Weight Management and Physical Activity Services in #Southend-On-Sea. These community-based, multi component services offer a personalised care package for adults, with a focus on those overweight or obese, and those who are at high risk of obesity and or inactivity. This service is delivered by an expert multi-disciplinary team, providing advice on healthy eating and physical activity. Playing a pivotal role within this service, you will be responsible for providing high quality, physical activity advice and support, including NICE Compliant Weight Management group delivery working within a multi-disciplinary specialist weight management team. You will provide expert physical activity advice, underpinned by a sound knowledge of theoretical principles of behaviour change, to support adults in Southend-On-Sea to make informed decisions in relation to their weight and or activity levels. The key functions of this role are: • Providing high quality physical activity advice and exercise prescription within a community-based, weight management service. • Delivering one to one and group interventions for adults who are overweight or obese, with potential co-morbidities/complex needs.  • Delivering group interventions for inactive adults.  • Working as part of a multidisciplinary team. An exciting opportunity is available for an enthusiastic and motivated stop smoking practitioner to join the Healthy Communities integrated lifestyle service for over 18’s in #Staffordshire team! The stop smoking practitioner is responsible for supporting and facilitating the delivery of smoking cessation programmes within the Healthy Communities integrated lifestyle service for over 18’s in Staffordshire, and Healthy Workplace Schemes. You will receive your own cohort of patients and face-to-face/virtual clinics (clinics run from Monday – Saturday, 9am-8pm) to support and maintain and will help to improve the accessibility of Staffordshire’s Stop Smoking Service through the pro-active promotion of the smoking cessation programme and referral pathways key health care providers, allied professionals, community events, local agencies, and the general public. We would like to hear from you if you: • Have strong interpersonal and communication skills. • Possess a minimum of 1 year experience working in a community-based smoking cessation service. • Are experienced in using behaviour change models within a work context. • Can demonstrate a clear understanding of the range and appropriate use of different smoking cessation intervention. • Display strong knowledge of the harmful effects of smoking, smoking as an addiction behaviour, withdrawal, and relapse. • Have experience in building strong links and relationships with a diverse range of groups, organisations, and agencies.
