As VR continues to gain traction, understanding its impact on students is vital for optimising training programs.   In a recent study at the Medical Faculty of Tübingen, Germany, 129 fourth-year medical students participated in VR-based emergency medicine simulations using the Oxford Medical Simulation software. The results were promising: 91% of students agreed that VR effectively conveyed complex medical scenarios, and 72% felt it could complement or even replace traditional mannequin-based training. However, the study also uncovered some interesting nuances.   Female students, in particular, expressed less enthusiasm for VR, finding it less useful and intuitive compared to their male counterparts. While most students appreciated the immersive experience (69%), empathy with virtual patients was low (16%), and confidence in the medical content was surprisingly minimal (3%). Notably, these gender differences in perception didn't translate into differences in test scores, although female students did report more physical discomfort during VR sessions.   These findings suggest that while VR holds great potential as a training tool, there may be a need to tailor VR experiences to ensure they are equally effective for all students. Additionally, the low confidence in the medical content presented in VR indicates that further refinement and integration of VR into the curriculum could enhance its educational value.   As VR continues to evolve, it’s clear that it could play a transformative role in medical education, but like any tool, its success depends on how well it’s implemented and adapted to meet the diverse needs of all learners.   #emergencymedicine #simulationtraining #simulationinhealthcare Ross Davies, Elliot Street, Jake Connell, Ben Quinton

In a groundbreaking study published in Nature, researchers explored the potential of AI to replicate the expert feedback typically provided by instructors during surgical training. The study involved 97 medical trainees who were divided into three groups: those receiving real-time AI feedback, in-person expert instruction, and no real-time feedback, all while performing a brain tumour resection simulation.   The AI system, known as the Intelligent Continuous Expertise Monitoring System (ICEMS), provided real-time auditory feedback to guide the trainees. Remarkably, the group that received #AI feedback outperformed both the expert-guided group and the control group in key areas like bleeding risk, tissue injury prevention, and instrument handling. These improvements were evident early in the training process, suggesting that AI can offer immediate, actionable insights that enhance learning.   Despite the AI group reporting a higher cognitive load, their performance remained superior, indicating that the system’s guidance was both challenging and effective. The AI-driven feedback also led to higher overall #OSATS scores, particularly in areas critical to surgical success.   This study highlights the potential of AI to revolutionise surgical education by providing precise, real-time feedback that rivals or even surpasses traditional expert instruction. With its ability to deliver consistent, objective assessments, AI could transform how surgical skills are taught, offering a scalable solution that ensures high standards across the board.   This research paves the way for integrating advanced technologies into medical training, ultimately aiming to improve patient outcomes and reduce errors in surgery.   What are your thoughts on the role of AI in medical education? Traditional methods have often fallen short in providing valid and reliable evaluations. Enter the Observational Clinical Human Reliability Analysis (OCHRA), a novel tool that shows significant promise in filling this gap. But how does it work, and what makes it different?   The Study: In 2012, a pioneering study, OCHRA was used to analyse 32 video-recorded laparoscopic colorectal resections performed by both expert surgeons and delegates of England's National Training Program (NTP). The study aimed to evaluate OCHRA’s construct and concurrent validity as a competency assessment tool.   Key Findings: • Error Analysis: A total of 399 errors were identified across the videos. OCHRA effectively differentiated between expert surgeons and less experienced delegates based on error counts. Experts had a median error count of 4, compared to 10 for those who passed the competency assessment and 17 for those who failed. • Error Types: Tissue-handling errors significantly distinguished between pass and fail groups (7 vs. 12 errors), suggesting that how tissue is managed during surgery is a critical indicator of competency. • Efficiency Measure (D/E Ratio): The ratio of dissection time to exposure time (D/E ratio) was another key metric. Experts showed a significantly higher D/E ratio (1.0 vs. 0.6), indicating greater efficiency in surgery. However, this ratio didn’t significantly differ between the pass and fail groups.   Conclusion: OCHRA stands out as a valid tool for assessing competency in advanced laparoscopic surgery at the specialist level. It not only differentiates between varying levels of expertise but also has the potential to be integrated into recertification and revalidation processes for surgeons.   Why It Matters: For the first time, we have a tool that offers an objective, detailed, and reliable assessment of surgical performance at the specialist level.
