It was a pleasure to welcome Blake Stephenson MP for Mid Bedfordshire, to Keech Hospice Care yesterday. With hospices facing ever-growing challenges from rising living costs and reduced funding, it was an important opportunity to show him the vital work we do. 🎉 Celebrating One Year of the Inclusive Health Outreach Service! 🎉 As of November 1, 2023, we are proud to mark one year of supporting Luton’s homeless community living with life-limiting conditions. In just one year, our service has achieved significant milestones: • Over 50 clients assessed for their psychological, social, emotional, and physical needs. • 150+ supportive contacts made with clients, care teams, and caregivers. • 40+ education and training events for healthcare professionals, students, and commissioners. • Ongoing collaboration with partners across primary, secondary, and voluntary sectors. We actively advocate for our clients through multidisciplinary team meetings, and we’re developing pathways to streamline their access to emergency services and hospital care. Our input into national policy highlights the critical need for equality in healthcare for the homeless community. Why is this service so vital? Many in our local homeless community face multiple life-limiting conditions and struggle to find adequate support. Your continued support has been essential in helping us grow and develop this service, and for that, we are incredibly grateful. The Inclusive Health Outreach Service is now 1 years old. 🎂 Since 1st November 2023 the Inclusive Health Service has continued to support the homeless community in Luton who are living with life limiting conditions. Since February 2024 over 50 clients have been supported and assessed for their ongoing psychological, social, emotional and physical needs. There have been over 150 supportive contacts to clients, supportive care teams and carers. There has been over 40 education and training events given for health care professionals, supportive care teams, healthcare students and also commissioners. Joint working and collaboration continue with the homeless partnership and the health care teams in primary, secondary and voluntary sectors. Attendance and advocating for client’s needs is ongoing through mult-disciplinary team meetings. There is ongoing development of pathways to help aid patients through accident and emergency services and hospital admissions. There has been input into national policy and reports and publication of articles promoting the need for equality in the homeless community. WHY DO WE CONTINUALLY NEED THIS SERVICE? This inclusive service is continually needed due to the large amount of our local homeless community struggling with multiple life limiting conditions and often struggling to find support with their symptoms and complex situations. Thank you to everyone for all your ongoing support in helping this service grow and develop. This service is only possible with your help and guidance, very grateful to you all.
