Last week we celebrated the publication of the The King's Fund report on our fantastic population health work in Greater Manchester. Working across organisations and communities is at the heart of everything we do – making Greater Manchester healthier for everyone. Here’s a wonderful example from #TerrificTrafford… Launching at the beginning of November and running through the winter months there is a new initiative called Winter Hearts being run at the Café in Stamford Park, Altrincham. This project is being led by The Counselling and Family Centre (CFC) who have close links with many community organisations, such as Age UK Trafford, GPs in Altrincham, and the local social prescribing team. Winter Hearts will be a community response to support older people at risk of health or social deterioration during Winter. We’ll post some links in our comments so you can read more #Trafford #IntegratedCare

Our local focus series has come full circle and we're back in 'Terrific Trafford' - the birthplace of the NHS. Throughout the week we'll be highlighting all the great work our partners are doing in the borough. Check out the Greater Manchester Health and Care Partnership Instagram and Facebook channels and tag us into any projects you'd like us to share Alternatively, pop a comment below. Some facts about Trafford: On 5 July 1948, Health Secretary Aneurin Bevan launched the National Health Service at Trafford General Hospital. It's home to the first industrial park in the world – Trafford Park. The Rolls-Royce engines that powered the Spitfire and Lancaster World War Two planes were built at Trafford Park. It's home to household names such as Kellogg’s, PG Tips and Adidas. At the north end of the borough, there are not one but two Old Traffords: Manchester United’s world-famous home ground and Emirates Old Trafford, the home of Lancashire Cricket Club. Famous people from Trafford include Karl Pilkington, Morrissey, Ian Curtis, Olympic gold medallist Darren Campbell and Diane Modahl.   #TerrificTrafford #Trafford #GreaterManchester #LocalityFocus Manchester University NHS Foundation Trust Trafford Council

We’ve recently celebrated Greater Manchester’s Good Employment Charter week (7 - 11 October). ‘Fair work is good work’ – we were invited to celebrate the difference and strengths that our communities bring to our workplaces, and a need to pay attention to the barriers that some colleagues face more than others. The week started with a launch of an interactive game of snakes and ladders to demonstrate how to understand what fair and equitable work is. John Herring and Sue Howard presented the Greater Manchester Multiple Disadvantage Framework to share practical ways workplaces can overcome the systemic barriers to work and help get people into sustainable employment. Mayor Andy Burnham joined the event to present a member’s plaque to us here at NHS Greater Manchester. Mark Fisher and John Herring are pictured here receiving the plaque.

We'd love health and social care teams across GM to join this webinar to find out about the latest Greater Manchester Care Record developments. Tameside GP Dr Saif Ahmed, our clinical adviser for the frailty network, will be delving into integrated care planning. The GM Care Record brings together patient information from NHS and care services across all ten Greater Manchester boroughs into one joined up record. This means information can be accessed by frontline health and care workers wherever the patient receives their care. Giving health and care workers access to patient information in the GM Care Record ensures patients receive the care and treatment they need, at the right time, in the right way. WEBINAR: Improving continuity of care through integrated care planning Personalised care planning in the GM Care Record brings a wealth of benefits to patients, transforming care from a reactive to a proactive service. Care plans provide a consistency of care – vital for patients facing long-term conditions such as dementia and heart failure. In addition to this, patients are able to access and contribute to their own care which offers an element of control and inclusivity. 🔗 Find out more here: https://bit.ly/4eB9aNH #weareHInM Graphnet Health

Greater Manchester is “the poster child for devolution” according to a report published today by The King's Fund. The report talks about the link between health and the communities we live in and praises our efforts to improve population health and to address health inequalities. Since 2015, Greater Manchester has had a wide-ranging devolution deal with Government on health which has led to improvements in life expectancy and other measures. One principle is at the heart of our devolution journey – more local decision making can deliver better outcomes for people. Greater Manchester Combined Authority https://lnkd.in/eerm8ErC

October is Freedom to Speak Up (FTSU) Month, which is all about raising awareness and empowering all staff to speak up about concerns in the workplace. The month is a reminder that we should feel able to say something - and be confident that our suggestion will be used as an opportunity for improvement. At NHS Greater Manchester, we’re committed to fostering a transparent environment that encourages all staff to be open and honest about their experience of work. Whether it’s about patient safety, the quality of care or staff well-being, your voice matters! FTSU is about providing a safe space in which people can speak up about anything that gets in the way of doing a great job. It helps us to identify areas for improvement and creates a culture of safety and support for both staff and the people we serve. Luzani Moyo is our lead FTSU guardian at NHS GM. He is one of our dedicated FTSU guardians and champions who are here to listen to the concerns of staff and guide them through the process. If you’re not comfortable speaking up directly, our anonymous tool provides a secure way to share your concerns. Our staff are also encouraged to complete the the national ‘speak up’ training module. Check out the FTSU page on the staff intranet for more information. #FreedomToSpeakUp #NHS
