    Right Procedure, Right Place: How OpenPredictor brings Innovation to the NHS    In the new report, Reforming elective care for patients – January 2025, we hear again how the NHS is under unprecedented strain. Wes Streeting and Amanda Pritchard highlight the depressing fact that over 6 million people are waiting for treatment, with many facing long delays. This is not just a statistic; it upends a foundational promise of timely care and gravely impacts people’s lives and livelihoods.    The report rightly states, “Continuing to do what we have been doing will not work: major reform to elective care is needed". As NHS surgeons, Mike and I saw firsthand how the burden on our service was growing. Doing more of the same wouldn’t suffice—we needed smarter, more targeted solutions. That’s why we founded OPCI and developed OpenPredictor, a clinical intelligence tool that uses machine learning to assess patients’ risk of post-operative complications.    The report also calls for solutions, that focus on “improving productivity by maximising the combined benefits of digital and operational improvements". OpenPredictor’s goal is simple but powerful: to get the right patients on the right surgical list in the right setting. By optimising pre-operative pathways, we’re helping hospitals to streamline processes, improve clinical outcomes, boost productivity, and ultimately lower costs.    In a time when reducing waiting times and clearing the elective backlog are critical priorities, we’re proud to be part of the solution.     "The shortage of anaesthetists prevents around 1.4 million operations and procedures from taking place each year. The RCoA warns this figure could increase to 8.25 million unless the government invests in training more anaesthetists because most operations cannot take place without one." These numbers highlight the urgent need to address the shortage of anaesthetists in the NHS, particularly as the service strives to meet government targets for reducing waiting lists for elective surgeries.     While training new anaesthetists is a necessary solution, with a training pathway of at least 14 years, action is needed now. Embracing innovative technologies to streamline the pre-operative process can play a pivotal role in optimising anaesthetists time and enhancing overall efficiency in the short term.     In one of the most hotly anticipated government announcements in years, Wednesday’s budget delivered key headlines for anyone involved in healthcare or health innovation. Knowing what we all do about elective surgery waiting lists, the repeated promise to cut waiting lists by 40,000 is welcome news, especially with the large increase in day-to-day spending committed. However, it will be interesting to see the detail how the government plans on achieving this number. Analysis by the Kings fund show  plans for further elective hubs will help, but we already know that there is pressure within the NHS to increase efficient use of existing surgical units. The key to this is productivity. One solution could be to invest in innovations and technology that streamline the pre-operative assessment process. With £2 billion of investment in NHS digital and technology promised, it seems there is significant scope for development in this area, especially combined with the commitment to maintaining R&D budgets within the NHS. Finally, it’s also good to see confirmation of the forthcoming Artificial Intelligence Opportunities Action Plan. There is huge potential in AI’s application to make improvements to UK Healthcare, but to be a benefit, and not a soon-forgotten fad, AI tools must be implemented with a clear strategy and proper governance and oversight.   • OPCI is live!    As NHS surgeons, we’ve been looking for new tools to help us improve patient care our whole careers. Over the past 5 years, we decided to apply computer science and machine learning to our field, orthopaedic surgery.    Through years of theorising, training and testing models we’ve developed OpenPredictor, a machine learning tool that supports clinician decision making in joint replacement pre-operative assessment and has the potential to have a transformative impact on elective surgery backlogs.    So far, we’ve secured significant funding and started a multi-centred pilot of OpenPredictor in Southwest Wales NHS Health Boards.
