🎉 TARGET REACHED!🎉 We love to share how our customers are doing and this snapshot of a practice in a part of the country with one of the highest prevalences of diabetes shows how SmartLife Health's searches tool can make a big difference in meeting your targets. Just a few months after using SmartSearches to help find and manage Type 2 diabetes patients, the practice has already reached their upper target for the nine key care processes and is now heading for the upper targets for 3TT and pre-diabetes. A huge well done to the practice team!

The number of patients for every permanent GP is rising with statistics showing the average GP in England has to care for 17% more patients than nine years ago. It means for every permanent GP there are more than 2,300 patients - a jump of nearly 350 since 2015. At SmartLife Health, we believe that good technology can help address part of this problem, allowing GPs and other clinical staff for focus on essential care and helping to address the growing health needs of the population. We don't promise to have the magic wand to fix primary care but we are playing our part in helping through better handling of clinical letters to smoother access for patients with long term conditions and insights into practice management. Janki Rao, Senior Clinical Pharmacist at Long Lane PCN, presents at the Royal Society of Medicine GP Update about how she and her team delivered award winning results for patients with Type 2 diabetes. It was a brilliant talk, which one member of the audience described as "inspiring" - we're proud of you, Janki. One of the tools which helped the team deliver their brilliant results was SmartSearches, part of the SmartLife Health portfolio. It helped find patients who should be being reviewed, risk stratified and provides a clear guide to teams for 'quick wins' such as patients with only one outstanding action in their 9 key care processes. It even helped with coding and QOF. And it's free for the first 100 practices who sign up. To find out more, visit www.smartlifehealth.co.uk

The UK Health and Security Agency has called on parents to check records after NHS data show not a single childhood vaccination met the threshold for herd immunity. SmartSearches for Childhood Immunisations is a simple way to monitor and plan your immunisation programme. You can see that one of our PCNs using SmartLife's software is making brilliant progress with their immunisation targets. The real time charts help practices plan their work and set up vaccination clinics to increase take up. It helps you protect more children against communicable diseases whilst achieving your national and local targets. Our Co-Founder, Dr Jay Verma, responds to the report by Prof Lord Darzi: "There is a huge amount of talent in this country creating answers to problems being faced across the NHS, most notably around clinical capacity and making sure we are using data correctly to prevent worsening health conditions. "Many of these providers operate in primary care, where there simply is not the time nor focused leadership to understand and implement what is available and what works. "Over 60% of NHS funding goes into hospitals and integrated care boards are hospital-heavy. This needs to change. "The best way to improve the nation's health and thus improve NHS is to focus on education and prevention. This is best done in primary care and in the community. "While a new laser might make for a more sensational news story, measures that reduce administrative burdens and use AI to identify high-risk patients—such as those with high blood pressure or diabetes—are likely to help more people and have a greater overall impact."

Ahead of the report by Lord Darzi on the NHS, which is expected to say that technology in the NHS is 15 years behind the private sector and there is too much focus on hospitals, we are resharing the mini manifesto of our co-founder, Dr Jay Verma on how to improve General Practice for all. #generalpractice #NHS #lorddarzi https://lnkd.in/eR_TRfbB

Today is World Suicide Prevention Day and it’s an opportunity to breaking down the stigma surrounding talking about mental health, including suicidal thoughts. Over 5,600 people committed suicide in England in 2023, equivalent to over 11 suicides per 100,000 people. In fact, for both males and females aged 5-34, intentional self-harm is the highest cause of death. Behind every one of these statistics is a person. A life. A tragedy. Research indicates that mental health conditions such as depression, psychosis and substance use are associated with an increased risk of suicide. SmartLife Health are providing our SmartSearches for SMI and CCMI for free for three months to help practices care for more patients suffering from poor mental health. People with SMIs are often difficult to reach and evidence shows they often neglect their health, resulting in poor health outcomes. SmartSearches for SMI/CCMI is designed to help practices contact their patients with these conditions and help teams monitor their progress, allowing multidisciplinary working. Please contact us to receive this free service and together we can help more people manage mental health. *SmartLife Health is offering a complimentary three-months of SmartSearches to practices that sign up and mention 'Suicide Prevention.' This initiative is designed to enhance the quantity and quality of patient reviews and treatment plans. #worldsuicidepreventionday #mentalhealth

❄ Preparing your patients for colder weather ❄ This autumn and winter, make sure your asthma patients can receive the best treatment and book their reviews simply and easily with SmartScheduler. This year, there is the added pressure of the removal of the winter fuel payment, which could leave vulnerable patients more at risk of exacerbations if they worry about the cost of heating their home. You can prioritise these patients with our SmartSearches tool for asthma and COPD and ensure they receive the care which can help prevent exacerbations or attacks. SmartScheduler allows you to contact all the patients with asthma and invites them to book in for a review at a time convenient to you and them. Reviews can be conducted easily, with patients at highest risk highlighted on the dashboard. Appointments can be carried out virtually or in person, and care plans completed and sent directly to patients. With SmartScheduler, everything is SNOMED coded back into the patient record, allowing your QOF register to be accurate and up to date.
