    Inside this blog is the The Agency for Healthcare Research & Quality (AHRQ) suggestions of three important steps for maximising and accelerating the transfer of research results. Traditionally, the main aim of clinical studies is to improve efficacy and safety results to guarantee the enhanced quality of life and patient outcomes.[1] However, variability of approaches in the practice of healthcare providers is observed. It is important therefore to consider the context in evidence-based practice. Strict criteria for inclusion and exclusion of participants in randomised controlled trials often do not reflect clinical reality.[2] Therefore, there is a need for additional evidence, such as real-world data studies and expert opinions, to cover the heterogeneity and diversity observed in real-world settings.[3] Medical Affairs is a rapidly evolving role that is increasingly required to demonstrate leadership within pharmaceutical organisations.     “The biggest reimagining of the NHS” since its creation in 1948 has been pledged by UK Prime Minister Sir Keir Starmer in response to a damning review of the healthcare system by Lord Ara Darzi (tinyurl.com/5ft3r2vx). The focus will be on three “shifts”—to prevention, community care and digital health. A top-down reorganisation of the NHS is not necessary, but more work is needed to “clarify roles and accountabilities” of the NHS headquarters, seven regions and 42 integrated care boards in #England.   The report—commissioned by the new government—finds that “successive governments have promised to shift care away from hospitals and into the community. In practice, the reverse has happened. Both hospital expenditure and hospital staffing numbers have grown faster than the other parts of the NHS, while numbers in some of the key out-of-hospital components have declined.” From 2006 to 2022, the share of the NHS budget spent on hospitals increased from 47% to 58%. Darzi notes that “single-year budgets necessarily reinforce the status quo—and when things go wrong the kneejerk response from ministers has been to throw more money at hospitals.”   The NHS is, in Darzi’s view, “in the foothills of digital transformation. The last decade was a missed opportunity to prepare the NHS for the future and to embrace the technologies that would enable a shift in the model from ‘diagnose and treat’ to ‘predict and prevent.’”   The report deplores chronic underinvestment and mis-investment in the NHS, with a backlog maintenance bill of more than £11.6bn and a shortfall of £37bn of capital investment. However, Starmer emphasised that the NHS would receive “no more money without #reform,” which will likely take close to a decade to complete.   There are few explicit references to the pharmaceutical industry, but the report does state that “there is enormous potential in AI to transform care and for life sciences breakthroughs to create new treatments.” Darzi believes the NHS must contribute to the nation’s prosperity and “should better support British biopharmaceutical companies.”   The study welcomes the fact that “medical and scientific advances means that disease can be better diagnosed and treated than ever before.” For example, since the turn of the century, the number of diseases with a known molecular basis has increased from 1,500 to nearly 7,500, while the number of gene mutations understood to cause disease has grown from around 1,000 to 5,000.   However, new medicines also present challenges: over the past decade, spending on drugs for specialised services and devices has grown at 8.9% and 10.2% per year, respectively. “This far outpaces the rate of growth of the total #NHS budget, meaning that specialised services account for a growing share of expenditure. While it means more diseases and conditions can be treated—such as putting England on a trajectory to eliminate hepatitis C ahead of the rest of the world—it creates an inexorable pressure on costs.”

    September is Sickle Cell Awareness Month Worldwide! 🩸 A lot more must be done to raise awareness and support those living with sickle cell disease, a genetic condition affecting millions worldwide. Effective management would involve earlier diagnosis, preventive care, and addressing complications with a multidisciplinary approach. However, despite advances in modern medical care that have improved outcomes, disparities in care still remain. Many individuals still face challenges with health management and their overall quality of life. We need to advocate for increased research, equitable and faster access to treatment options, and greater education on the realities of living with sickle cell. Triducive believe that the voices of both patients and experts MUST be heard to catalyse real change. We hope to see more evidence-based consensus raised around this demanding disease in the coming years. #SickleCellAwarenessMonth #HealthcareAdvocacy #GeneticDisorders #SupportSickleCell
