We always love reading Ashleigh Fox's insightful articles. The Bridging Support model, piloted back in 2017, continues to show that with dedication and collaboration, bringing people home is possible. It truly takes a village or, in our case, #SystemsThinking. Thanks for sharing, Ash! Read the article below:

Monday morning thoughts on #homesnothospitals. What are the practical things we can each do? We’ve learned it can be done with commitment from everyone involved, through building and piloting the Bridging Support model back in 2017 through LD Network and Nurseline Healthcare, as well as our other provider organisations Leaf Complex Care, Nurseline Community Services and Unique Community Services as #SmallSupports to bring people home. Let's chat - it takes a village. Or in our case, #systemsthinking. Love, Ash #autism #learningdisability #commissioning #socialcare #NHS #humanrights

William is one of the people we support, a 20-year-old young man who loves spending time outdoors, playing word games, swimming, and bouncing on a trampoline. William visits the park every day, enjoys meals out at restaurants, and has a deep fascination with trains. Each summer, William heads to Scotland, where he spends time with his extended family, exploring the seaside and walking in nature. Over the years, William has developed strong bonds with his carers, allowing him to enjoy his holidays with greater freedom and happiness. This trust has also enabled him to embrace new activities and feel more confident in different settings. Watch his story and find out more about his unforgettable holiday this year ➡️ https://lnkd.in/dz7zptjY Catalyst Care Group #uniquecommunityservices #complexcare #holidaysupport

To our incredible team—this recognition reflects your dedication, compassion, and tireless effort. Thank you for always going above and beyond. We couldn't be prouder to stand alongside you. Here's to continuing to make a positive impact together. #CareAwards2024 Care & Support West Catalyst Care Group
We are proud to announce that the Unique Community Services team is a finalist at the #CareAwards2024 at Care & Support West for Care Organisation of the Year. This recognition highlights their dedication to breaking down barriers and transforming lives. Recently, the team supported an autistic young man and his family on a life-changing holiday to Scotland. The family enjoyed their first uninterrupted holiday in six years, thanks to meticulous planning and compassionate care. The impact was profound, as the young man engaged in activities like sea swimming and park visits, and his family experienced a much-needed sense of peace and normalcy. As his father said, "It's the first time in 8 years we've had a genuine rest." To the Unique team, we couldn't be more proud of the impact you've made. Good luck tonight! 🤗
