Eu sou do tempo do RUP, sendo que não concordo com essa visão linear, para mim ela é tão assim quanto qualquer outra, ser for bem aplicada. Assim como vejo pessoas pegando o Scrum e burocratizando, complexando e achando que vai conseguir os resultados originais. Então não coloque a culpa do resultado na metodologia, ele foram criadas para um propósito e funcionam se você receber e seguir. Dominou a original, agora sim você pode fazer ajustes, pois já tem autoridade pra isso e saberá se piorou ou melhorou o resultado. Estude, troque experiências sobre e o principal aplique, veja que todas em algum nível aplicam as bases do PDCA, então são como uma maratona, são simples, mas não são fáceis e nem para qualquer um.

Al considerar la naturaleza del proyecto se podrá identificar la metodología que utilizarás. La especificación de requerimientos,sea con la metodología que sea debe ser clara, concisa, no ambigua, flujo principal claro, flujo secundario y/o excepciones indetificables. Lo recomendable es mantener un formato simple de documentar estos requerimientos y un mapa conceptual de la solución que se espera, también lo puedes acompañar con la arquitectura de información (altamente recomendable)

Defina um limite para receber respostas para evitar "chegar junto com o e-mail", se comunique de forma assíncrona sempre que possível. Evite chamar para reuniões pessoas que não vai agregar valor, deixe as trabalhando. Troque reuniões por emails ou mensagens em ferramentas de comunicado e por último cuidado com os excessos para não perder relevância e vira spam

Ensuring team members understand the role they play on the project will save you time and trouble in several ways. By clearly stating roles (either using RACI or another method) you call out each team member as a valuable asset and set the stage for other team members to connect where overlap exists. Additionally, each team member brings different skills and experience to a project, you may have a Sr member who thinks they know it all and new hires that are there to learn. Both are valuable assets to a project and by defining the roles clearly, you can give each person a place to shine.

Just doing a retro will not make the next project/phase better, it is in the shared learning and agreed adaptation of our approaches and methods that we become more and more successful.
