How can medical schools integrate empathy training into their curricula?

Empathy cannot be taught, but can be practiced. I would suggest having once a week mini group sessions among students that can go from 30-60 minutes before starting any formal academic sessions in which they can discuss very briefly their current emotional status, concerns, joys or thoughts.

Medical texts that teach about palliative care and end of life discussions would be a start. Physicians who avoid speaking truths can not be empathetic. Period. Truth is not an attack; it’s an open opportunity to collaborate with medical facts. It’s not empathetic to avoid discussing a patient’s weight when they are prescribing meds (with toxicities) to cover up obesity issues. It’s not empathetic to avoid discussing palliative care planning for patients with a fatal diagnosis. Train physicians to collaborate with their patients through medical facts. That’s your first step.

Have them spend 48 hours in a wheelchair. No time off when getting up would make things easier. This humbling experience would change the perspective of those clinicians by enhancing the communication, collaboration, professionalism and most importantly empathy. To have to walk or in this case roll a mile in someone else’s shoes may reduce the issues associated with does effect the quality and equity of care.

Very insightful topic. Many of us having been managing the end of life for loved family members and close friends at this point in our lives. I think having "truth in treatment" would bring an immeasurable quality of life for the patient and cargiver (aka most of us at some point). The discussion, although incredibly hard, when done earlier brings a quality of life and makes the journey to end more "peaceful and meaningful". I am an Echocardiographer, noticing that recent generations are limited in their empathy. It may not always come from a place of true feeling for the patients situation, but from what will get an echo done quickly, in order to move on to the next. Perhaps externships could also consist of spending time with nurses, especially in the Emergency room. People come in often in dire circumstances, and can only be themselves-raw and unfiltered. There may be a lot to gain from a somewhat humbling experience. Some may disagree- this is not the focus of echo, but it is so important to put yourselves in patients shoes at times-just might end up making the echo go quicker if they are understanding of the patients needs, both physically and emotionally.

Empathy, a crucial human trait, is not something that can be effectively instilled through formal training alone. It is cultivated through life experiences and the nurturing environment of one's upbringing and education. This emotional skill grows and deepens as we interact with diverse individuals and situations throughout our formative years. If one waits until entering a professional setting, like medical school, to begin developing empathy, they may find it challenging to fully integrate this essential attribute into their professional practice. True empathy requires a lifetime of learning, understanding, and connecting with the emotional experiences of others.

There are several major obstacles to adding empathy training in medical school to consider, such as curriculum overload. Medical school curriculum are already extremely busy and piling on more to a students schedule would be difficult to manage without overhauling the way we view medical school foundational. Additionally, the high-stress environment of medical training may hinder the development and expression of empathy as many students keep their thoughts and feelings close to the vest.

In my opinion there are two specific moments to facilitate empathy training in faculties. First of all, the way of choosing candidates to pursue a medical profession must be more focused on soft skills, because today the problem of knowledge is almost solved with the number of tools such as AI and the globalization of knowledge. We don't need more people memorizing recipes, but rather trying, listening and understanding people's suffering. And on the other hand we need more empathetic teachers, the way of learning, especially the way of managing the vocation, will depend on the way of teaching, so it is important to have more empathetic teachers who really inspire medical students.

Challenges in integrating empathy training include time constraints within a packed curriculum, varying levels of student engagement, and the need for ongoing reinforcement to ensure empathy skills are consistently applied in clinical settings. Faculty support and resources also play crucial roles in overcoming these challenges.

The best way to teach empathy is to have a student go through the same experience as the patient they are trying to feel empathy for. We can do this with mock scenarios. These mock scenarios should include having to schedule appointments, understanding the wait times, sitting in awaiting rooms, and yes, even the experience of staying in a hospital bed overnight. Included should be interviews with patients with chronic diseases, cancer, heart disease, and trauma, to learn how these patients deal with their diagnoses and how they manage to navigate the healthcare system. This combination of sharing patients’ experiences and listening to patients who are managing health issues would be a memorable and important learning method.
