Collecting statements from all involved in an incident helps gather diverse insights and a template ensures all required information is captured effectively. Determining the sequence of events from the beginning of the incident until the emergency team's arrival aids in identifying the root cause and contributing factors. Photos offer visual evidence, revealing overlooked details from initial evaluations. They consist of wide and close-up shots to showcase the extent of damage. These practices are essential in comprehensive incident investigation and prevention strategies.

A typical scenario. A mechanic working on the gearbox of a Toyota Land Cruiser pickup, had 2 employees lift the gearbox from the cabin with slings while he and another mechanic were underneath the vehicle guiding the casing (supported on a hydraulic jack) to align with the clutch and then couple it. He called for the team to rest a second before pushing the casing into place. The guys in the cabin then left the slings and all looked fine for them to take the rest until suddenly the casing shifted and dropped, consequently grazing the mechanic on the forehead. He sustained cuts on the forehead and the left eyebrow. Two sutures for each cut and he was given excused duty. Question is, is this an LTI or an MTI?

We need to make sure ,correct identification of incident which is actually a base of whole Process which is called Incident Management.

Also, evacuating crows is very important to ensure that lives and property are protected and that no other dangers are incurred to those present at the work site

Don't leave Incident Scene unattended until investigation Completed, we can use different means to collect data but the most important is witness/victims statement.

Make sure that the identities of the witnesses and clips taken from the CCTVs are secured in a way that preserves the rights of everyone without exposing them to any direct or indirect peer pressure, racial or biased pressures to take feedback from them in a quick, safe and impartial manner

One thing I find helpful is also to avoid ambiguity when collecting objective evidence to support your data. Also confidentiality where needed, is very important.

A safe work environment is one that is completely free from criticizing and blame. Make sure things are as practical and professional as possible

One of the things that help have a comprehensive understanding of the causes is to take small steps in the causal tree. The bottle did not break because it was toppled off the table, it broke because of the impact to the ground. This way, you can recognize more causes that you might be able to fix with actions. After the incident, one secured the scene and collect the evidence etc. However, writing a report does not need to write a summary or introduction in it. Every organisation have a standardized format (forms) through which the safety professional writes their reports of incidents and submits them to the client.

Overall it's correct. But it failed to identify the human factor and include the necessary recommendation of Documenting: 1. Personnel involved 2. Emergency Response Plan and how/where Injured Person being treated, status etc 3. Overall it's a guideline but lack of concrete perception in prepping these documents for further Legal Responsibilities / Court Hearings

In my jurisdiction, the report format used is the ICAM and it's pretty simple and convenient to use. Brings out the picture without you being a pro painter. That is to say, filling the ICAM with the data gathered produces a near perfect report without you being exceptional in report writing.

This all sounds great but in practice. There are many competing tasks for your time. Problem alot of incidents are not closed because time it takes to Get all stake holders updated. Need to ask yourself, How can you make following up more efficient and time effective? Dealing with incidents can be a full time job.

In Follow up plan we need to be realistic, we should keep in mind all points which a company/department may face and purpose is to make sure all control measures implemented as intended.

Organization should have incident/ accident reporting and investigation system as part of their HSE management system including near misses. Which should be documented and reviewed/ updated periodically. Elements identified are part of such documents.

Something to consider, and I am not sure if it has been mentioned, but timelines plays a major factor in reporting. This doesn’t mean you hurry through the investigation to finalize your report, but understand the importance of getting these documents to your “Safety Team” and how it can impact your organization.

As a corporate HSE team, we would like to perform Disaster Management & EM response study and document the EM incident criteria, identify EM team and conduct internal training and mock drills. This would enable and allow the personnel at the facility to build awareness and prepare. It is also necessary to nominate EM Captains at strategic locations and empower them to act in the event of emergency. It is essential that all EM incidents are recorded in a prescribed format and reviewed in the next internal QMS meeting as lessons learned. Plough back in EM incidents and the identified layers of protection & Safeguards in the DMP plan documents.

Training and identifying key people are essential. Not just raising hands who has CPR in a meeting, but people who accept this as an additional role in their work routine. I have seen workplace emergency’s fall apart. Tools, pre-plans and ERPs are great but emergency response execution with trained personnel is a successful combination.
