By doing so, you can monitor progress towards goals and outcomes, identify any barriers or challenges in the delivery of services or supports, adjust interventions or resources to address issues or opportunities, celebrate achievements or improvements of the client, involve the client in decision-making and planning, and maintain a positive and trusting relationship with them and other stakeholders.
