In my experience, it all boils down to identifying the lesser evil. Obviously, no problem is too small to be ignored. But the one having the least impact has to be put at the last in the list of priorities. Next step is to set the timelines for each deliverable which need to be set by consensus and should be adhered to by all the parties. Eg: In an ERP system, anything that impacts employee pay has to take priority over say something that impacts user experience.
