Many of the scenarios where cross-functional teams are formed related to a specific A-B transformation. As such, clear and agreed top-line goals are important but equally important is agreeing who is accountable for areas of the transformation - project management, delivery of components, review/checkpoints. Where the accountability isn't clear or is too distributed, my experience is that optimisation isn't achieved, either in delivery or achievement of objectives.

You need to deal with conflict either way (see #4). By setting clear lines up front, you can give the team more latitude to negotiate conflict, referring to that original charter when things start to go sideways. Also, remember that these teams will often need to reshape themselves over time. Keep in touch with changes in scope and membership. Continuous improvement requires continuous feedback, and open communication should be a thread binding all stakeholders. It's a big investment. Make sure everyone stays on board.
