One excellent book that's inspired us at Genesis Orthopedics & Sports Medicine is called "Reverse Innovation in Health Care: How to Make Value-Based Delivery Work" by Vijay Govindarajan and Ravi Ramamurti. I'd highly recommend it for an excellent treatment of lowering operational costs for the purpose of expanding access.
