Thus, have a risk list, risk matrix and risk management plan, since with all respect to individual manager awareness and workshop certificates, such risks are job, location and time bound. So, if all appropriate contingency and response was provided and still the risk materialized (failure occurred), record it in your Lessons Learnt book and Organizational Process Assets and see what should be improved in risk management procedures (contingency size, probabilities etc.).
